注射用泮托拉唑联合胃内镜下止血治疗上消化道出血的有效性和安全性

Efficacy and safety of pantoprazole for injection combined with hemostasis under gastroendoscope in the treatment of upper gastrointestinal bleeding

目的观察注射用泮托拉唑联合胃内镜下止血治疗上消化道出血患者的有效性和安全性。方法选取2016年6月-2019年8月福建医科大学附属三明第一医院收治的上消化道出血患者70例,采取随机数字表法分为观察组和对照组,每组35例。对照组采用注射用泮托拉唑联合去甲肾上腺素治疗,观察组采取注射用泮托拉唑联合胃内镜下止血治疗,比较2组治疗效果、胃内pH值改善情况及不良反应。结果观察组总有效率为97.14%,高于对照组的71.43%(χ2=8.737,P=0.003);治疗后,观察组胃内pH值为(6.6±0.4),高于对照组的(5.6±0.5)(t=9.239,P=0.000);观察组不良反应发生率为2.86%,低于对照组的17.14%(χ2=3.968,P=0.046)。结论上消化道出血患者采取注射用泮托拉唑联合胃内镜下止血治疗效果显著,可有效提高患者胃内pH值,安全性较高,值得临床推广应用。

Objective To analyze the efficacy and safety of pantoprazole for injection combined with hemostasis under gastroendoscope in the treatment of upper gastrointestinal bleeding. Methods 70 patients with upper gastrointestinal bleeding admitted to Sanming First Hospital Affiliated to Fujian Medical University from June 2016 to August 2019 were randomly divided into observation group and control group,35 cases in each group. The control group were treated with pantoprazole for injection alone. For treatment,the observation group were treated with pantoprazole plus endoscopic hemostasis. Compare the treatment effects,the improvement of gastric pH and adverse reactions between the two groups. Results The total effective rate of the observation group was 97. 14%,which was higher than 71. 43% of the control group(χ~2= 8. 737,P = 0. 003). After treatment,the intragastric pH value of the observation group was(6. 6 ± 0. 4),which was higher than that of the control group(5. 6 ± 0. 5)(t = 9. 239,P = 0. 000). The incidence of adverse reactions in the observation group was 2. 86%,which was lower than 17. 14% in the control group(χ~2= 3. 968,P = 0. 046). Conclusion The application of pantoprazole for injection and endoscopic hemostasis in patients with upper gastrointestinal bleeding is effective,safe and has significant clinical value.
