摘要
目的:通过对生理性平脉、生理性弦脉及高血压弦脉患者双侧颈总动脉、桡动脉、足背动脉瞬时波强参数瞬时加速度波强(W1)、瞬时减速度波强(W2)及负向波面积(NA)的对比分析,初步探讨生理性弦脉及高血压弦脉的能量学变化特点,以期为脉诊研究增加能量学方面的信息。方法:应用ALOKA Prosoundα7彩色多普勒超声诊断仪对52例生理性平脉、54例生理性弦脉、55例高血压弦脉患者的颈总动脉、桡动脉及足背动脉的瞬时波强参数进行探测及统计分析。结果:生理性平脉组与生理性弦脉组颈总动脉、桡动脉及足背动脉处的W1、W2及NA值差异均无统计学意义(均P>0.05),上述2组颈总动脉、桡动脉、足背动脉处的W1值及颈总动脉、足背动脉的NA绝对值均低于高血压弦脉组,差异均有统计学意义(均P<0.01)。生理性平脉组桡动脉处的NA值及颈总动脉、桡动脉处的W2值均低于高血压弦脉组,差异均有统计学意义(均P<0.01)。结论:高血压弦脉组的能量学特点为收缩期处于代偿性高能量状态。瞬时波强技术可直接为脉象客观化研究提供能量学方面的客观依据,并能拓展脉诊部位,可更全面地反映脉象特点,为脉象客观化研究提供更丰富的信息。
Objective:To explore the characteristics of energy changes of physiological and hypertensive taut pulse by comparing the wave intensity(WI)parameters W1,W2 and negative area(NA)of bilateral common carotid artery,radial artery and dorsalis pedis artery in subjects with normal pulse,physiological taut pulse and hypertensive taut pulse,so as to increase energy information for pulse diagnosis research.Methods:52 subjects of normal pulse,54 subjects of physiological taut pulse and 55 subjects of hypertensive taut pulse were enrolled and Prosoundα7 color Doppler ultrasonography of ALOCK company was used to detect bilateral common carotid arteries,bilateral radial arteries and bilateral dorsal arteries of WI technology parameters.Results:There were no significant differences in W1,W2 and NA values between normal pulse group and physiological taut pulse group(all P>0.05).The NA value of radial artery,W2 value of common carotid artery and radial artery in normal pulse group were lower than those in hypertension taut pulse group(all P<0.01).Conclusions:The energy characteristic of hypertensive taut pulse group is that during systolic period the body is in a compensatory high energy state.WI technology can directly provide the objective basis of energy information for the objective study of pulse,and can expand the position of pulse diagnosis,reflect the characteristics of pulse more comprehensively,and provide more information for the objective study of pulse.
