18F-FDG PET/CT显像特征与多发性骨髓瘤患者临床因素的相关性分析 被引量:5

Correlation analysis of F-FDG PET/CT imaging characteristics with clinical characteristics of multiple myeloma patients

摘要 目的:正电子发射型计算机断层显像/计算机体层成像(PET/CT)在多发性骨髓瘤(MM)中的应用日渐广泛。本研究探讨初治MM患者的18F-脱氧葡萄糖(18F-FDG)PET/CT影像学特征与临床特征的相关性。方法:收集2016-01-2019-12我院血液科收治的在治疗前行PET/CT检查的初治MM患者的基本资料,包括骨病类型及最大标准化摄取值(SUVmax)等PET/CT影像特征及临床指标。采用SPSS 26.0统计软件进行统计学处理。结果:57例初治MM患者,男女患者比例为1.19∶1.00,平均(66.0±9.7)岁。其中42例伴骨质破坏患者的骨质破坏区SUVmax>4.75者24例(57.1%)。18例伴髓外病灶患者中髓外病灶SUVmax>4.00者14例(77.8%)。Mann-Whitney U检验提示,患者不同骨病类型间DS Plus分期(P=0.027)及年龄(P=0.041)差异有统计学意义。骨破坏区的SUVmax与骨髓浆细胞比例(P=0.017)、ISS分期(P=0.008)、血清白蛋白(P=0.009)及β2微球蛋白(P=0.008)呈相关关系。髓外病变SUVmax与乳酸脱氢酶呈正相关(P=0.021)。结论:PET/CT能在一定程度上反映MM患者的临床特征,可能对于初治MM患者的预后有参考价值。 Objective:Positron emission tomography/computerized tomography(PET/CT) is widely used in multiple myeloma(MM).The purpose of this study was to investigate the correlation between 18F-fluorodeoxyglucose(18F-FDG) PET/CT imaging characteristics and clinical characteristics in primary MM patients.Method:The data of newly treated MM patients admitted to the Department of Hematology from January 2016 to December 2019 were collected,and all patients had undergone PET/CT examination before treatment.The aim was to summarize the clinical characteristics of patients and PET/CT image characteristics,including the type of bone disease and maximum standardized uptake value(SUVmax).We used the statistical software SPSS 26.0 to analyze the results.Result:There were 57 newly-treated MM patients in this study,of which the ratio of male and female patients was 1.19∶1.00,with an average of(66.0 ± 9.7) years and a median age of 68 years.Of the 42 patients with bone destruction,24 cases(57.1%) with SUVmax> 4.75 in bone destruction area.Among the 18 patients with extramedullary lesions,14 cases(77.8%) with SUVmax>4.00 in extramedullary lesions.Mann-Whitney U test showed that there were statistical differences in DS Plus staging(P=0.027) and age(P=0.041) among patients with different bone disease types.SUVmax in the bone destruction area was correlated with the proportion of bone marrow plasma cells(P=0.017),ISS stage(P=0.008),serum albumin(P=0.009),and β2 MG(P=0.008).SUVmax was positively correlated with lactate dehydrogenase in extramedullary lesions(P=0.021).Conclusion:PET/CT can reflect the clinical characteristics of MM patients to a certain extent,which may have reference value for the prognosis of MM patients treated initially.
