胆汁色度卡联合自我管理单在带经皮经肝胆道穿刺引流管患者延续性护理中的应用

Application of Chromaticity Card Combined with Self-management in Continuous Nursing of Patients with PTCD Tube

目的探讨色度卡联合自我管理单在带经皮经肝胆道穿刺引流(PTCD)管患者延续性护理中的应用。方法选取2017年6月—2018年6月带PTCD管出院患者90例,按照随机数字表法分为对照组45例和观察组45例,两组患者于住院期间和出院前都由责任护士行带PTCD管护理方法和出院后注意事项进行健康教育,出院后一周予电话回访。进入观察组的患者在出院时给予一张胆汁颜色色度卡和自我管理单带出院,居家时自行填写自我管理单,出院两周后回院复查时回收。结果观察组居家时自我管理情况,正确记录引流液的颜色、数量、性状有41例,妥善固定引流管有35例,定时更换引流袋的有39例,定时换药有39例;在并发症方面,发生感染5例,管道脱出2例,管道堵塞3例。与对照组进行对比,差异有统计学意义(P<0.05)。结论色度卡联合自我管理单的应用,患者能够直观的分辨胆汁的颜色,正确护理管道、及时换药和更换引流袋,降低居家时并发症的发生,有利于患者顺利康复。

Objective To explore the application of chromaticity card combined with self-management in continuous nursing of patients with PTCD tube.Methods 90 patients discharged with PTCD tubes from June,2017 to June,2018 were selected.According to the random number table,they were divided into control group and observation group,with 45 cases in each group.Both groups were treated with PTCD tube by the responsible nurse during hospitalization and before discharge.Return visit by telephone one week after discharge was done.Patients enrolled in the observation group were discharged with a bile chromaticity card and self-administered single band,the self-management form at home was filled,and review of two weeks after discharge back to the hospital was recycled.Results In the observation group,self-management at home was observed.41 cases correctly recorded the color,quantity and character of drainage fluid,35 cases properly fixed drainage tube,39 cases regularly changed drainage bag and 39 cases regularly changed dressing.In terms of complications,there were 5 cases of infection,2 cases of pipe prolapse and 3 cases of pipe blockage.Compared with the control group,the difference was statistically significant(P<0.05).Conclusion Colorimetry card combined with self-management single application,patients can intuitively distinguish the color of bile,correct nursing pipeline,timely drug change and change drainage bag,and reduce the incidence of complications at home.It is conducive to the smooth recovery of patients.
