小切口直视微创冠状动脉旁路移植术的麻醉管理 被引量:1

摘要 小切口直视微创冠状动脉旁路移植术(Minimally invasive direct coronary artery bypass,MIDCAB)主要是指经左胸小切口、右胸小切口或胸骨旁小切口行左乳内动脉到左前降支或右乳内动脉到右冠状动脉主干的不停跳冠状动脉搭桥术。1996年10月阜外医院和北京邮电医院分别实施胸骨旁小切口和左前外小切口的MIDCAB,开创了我国MIDCAB的先河。20余年来,MIDCAB在国内开展日益广泛,它的适应证从单支病变扩展到多支病变,由于其不受场地不限制,无需胸腔镜、机器人等高端技术支持,学习曲线短,无需劈开胸骨,创伤小,疼痛轻,无需输血,并发症少,术后恢复快,总住院费用少等诸多优点,得到越来越多外科医生及患者的认可。MIDCAB因其手术操作空间有限,在非体外循环下、跳动的心脏上进行的,更易发生严重心血管意外且不易处理,需要更精细、更复杂的规范、安全有效的麻醉管理,这对麻醉医生带来了很大的挑战。本文将就临床上该术式的麻醉管理进行总结,并对其未来发展做出展望。
