子宫动脉结扎术联合卡贝缩宫素预防双胎剖宫产产后出血的研究

Preventive effect of uterine artery ligation combined with capecitabine on bleeding after cesarean section in twins

目的研究子宫动脉结扎术联合卡贝缩宫素预防双胎剖宫产术产后出血的临床效果。方法选取2019年2月至2020年3月本院收治的双胎妊娠行剖宫产的术中胎儿娩出后予缩宫素10 U宫体注射,胎盘娩出后观察有产后出血倾向的孕妇100例。随机分为实验组和对照组,每组50例。实验组行双侧子宫动脉结扎术迅速阻断子宫出血,手术结束前给予卡贝缩宫素100μg预防产后出血;对照组按常规产科处理。比较两组产妇产后出血发生率及术中术后出血量。结果实验组产后出血发生率为0.00%,明显低于对照组的8.00%(P<0.05)。实验组术中、术后2、24 h出血量均低于对照组(P<0.05)。结论子宫动脉结扎术联合卡贝缩宫素对于预防双胎剖宫产术后出血效果明显,可有效降低产后出血发生率,提高治疗效果,临床价值较高。

Objective To study the clinical effect of uterine artery ligation combined with capecitabine on preventing bleeding after cesarean section in twins.Methods From February 2019 to March 2020,twin pregnancies who underwent cesarean section in our hospital were selected.After delivery,100 cases of pregnant women with postpartum hemorrhage tendency were observed.They were randomly divided into experimental group and control group,with 50 cases in each group.The experimental group was treated with bilateral uterine artery ligation to quickly block uterine bleeding,and 100μg carbetoxin was given before the end of operation to prevent postpartum hemorrhage;the control group was given routine obstetric treatment.The incidence of postpartum hemorrhage and the amount of intraoperative and postoperative bleeding were compared between the two groups.Results The incidence of postpartum hemorrhage in the experimental group was 0.00%,significantly lower than that in the control group(8.00%,P<0.05).The amount of blood loss in the experimental group was lower than that in the control group at 2 and 24 hours after operation(P<0.05).Conclusion Uterine artery ligation combined with kabeoxytocin has obvious effect on preventing postpartum hemorrhage after twin cesarean section.It can effectively reduce the incidence of postpartum hemorrhage and improve the therapeutic effect,it has high clinical value.
