EMBM治疗食管癌前病变与早期食管癌患者的临床疗效及预后效果

The clinical efficacy and prognostic effect of EMBM in the treatment of patients with precancerous lesions and early esophageal cancer

目的研究多环黏膜套切术(EMBM)治疗食管癌前病变与早期食管癌患者的临床疗效及预后效果。方法选取本院2017年11月至2019年2月收治的食管癌前病变与早期食管癌患者110例,采用随机数字表法分为观察组与对照组,各55例。对照组行内镜下黏膜切除术(EMR)治疗,观察组行EMBM术治疗。对比观察两组患者术后效果、生活质量及患者术后并发症发生情况。结果观察组治疗总有效率(98.18%)高于对照组(69.09%),术后总并发症发生率(7.73%)低于对照组(21.82%),差异均有统计学意义(P<0.05);观察组术后躯体、心理、社会、环境及综合评分均高于对照组,差异有统计学意义(P<0.05)。结论EMBM治疗食管癌前病变与早期食管癌具有操作简便、用时短、术后并发症发生率低、安全性高、疗效确切等优势,且能提高患者术后生活质量,值得临床推广。

Objective To study the clinical efficacy and prognostic effect of multi-loop mucosal tracheotomy(EMBM)in the treatment of patients with precancerous lesions and early esophageal cancer.Methods A total of 110 patients with esophageal precancerous lesions and early esophageal cancer admitted to our hospital from November 2017 to February 2019 were selected and they were divided into observation group and control group according to random number table method,with 55 cases in each group.The control group was treated with endoscopic mucosal resection(EMR),and the observation group was treated with EMBM.The postoperative effects,quality of life and postoperative complications were compared and observed between the two groups.Results The total effective rate of treatment in the observation group(98.18%)was higher than that of the control group(69.09%),and the total postoperative complication rate(7.73%)was lower than that of the control group(21.82%),the difference was statistically significant(P<0.05).The postoperative physical,psychological,social,environmental and comprehensive scores of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion EMBM treatment of precancerous lesions and early esophageal cancer has the advantages of simple operation,short time,low incidence of postoperative complications,high safety,and definite curative effect.It can improve the quality of life of patients after surgery and is worthy of clinical promotion.
