低分子肝素联合地屈孕酮治疗子宫动脉血流阻力升高型复发性流产的保胎效果观察

Observation on the effect of low molecular weight heparin combined with didroxyprogesterone in the treatment of recurrent abortion

目的研究低分子肝素联合地屈孕酮治疗子宫动脉血流阻力升高型复发性流产的保胎效果。方法选取本院2018年7月至2019年12月收治的50例子宫动脉血流阻力升高型复发性流产患者,以随机数字表达法分为观察组和对照组,每组25例。对照组口服地屈孕酮片,观察组在对照组基础上加用脐周皮下注射贝米肝素钠。观察两组患者子宫动脉血流阻力变化情况,比较妊娠活产率及药物不良反应。结果观察组PI、RI、S/D均低于对照组(P<0.05),且观察组血小板减少、阴道流血、肝酶异常等不良反应发生率,与对照组比较差异无统计学意义,而观察组妊娠活产率明显高于对照组(P<0.05)。结论低分子肝素联合地屈孕酮可有效降低子宫动脉血流阻力,降低流产风险,保胎成功率较高,且药物安全性高,在临床中有较高的应用价值。

Objective To study the effect of low molecular weight heparin(LMWH)combined with didroxyprogesterone(DDP)in the treatment of recurrent abortion.Methods 50 cases of recurrent abortion with increased resistance of uterine artery blood flow from July 2018 to December 2019 were selected.All patients were divided into observation group and control group,with 25 cases in each group.The control group was given didroxyprogesterone tablets,and the observation group was given Bemiparin sodium injiection subcutaneously around the umbilicus on the basis of the control group.The changes of uterine artery blood flow resistance were observed in the two groups,the live pregnancy rate and adverse drug reactions were compared.Results PI,RI and S/D in the observation group were all lower than those in the control group(P<0.05),and the incidence of adverse reactions such as thrombocytopenia,vaginal bleeding and abnormal liver enzymes in the observation group had no statistical significance compared with the control group,while the live birth rate of pregnancy in the observation group was significantly higher than that in the control group(P<0.05).Conclusion Low molecular weight heparin combined with didroxyprogesterone can effectively reduce the resistance of uterine artery blood flow,reduce the risk of abortion,have a high success rate of fetal protection,and high drug safety,and have a high clinical application value.
