新型膀胱冲洗引流装置对持续膀胱冲洗患者尿路感染及尿液菌落数的影响 被引量:9

Effect of new bladder irrigation and drainage device on urinary tract infection and urine colony count in patients with continuous bladder irrigation

摘要 目的探究新型膀胱冲洗引流装置对持续膀胱冲洗患者尿路感染及尿液菌落数的影响。方法回顾性分析右江民族医学院附属医院2017年7月-2019年6月82例行前列腺等离子电切术(Plasmakinetic resection of prostate,PKRP)治疗患者的临床资料,其中2017年7月-2018年6月39例患者经传统膀胱冲洗引流装置持续膀胱冲洗(对照组),2018年7月-2019年6月43例患者经新型膀胱冲洗引流装置持续膀胱冲洗(研究组)。记录两组持续膀胱冲洗时间、冲洗液清亮时间、冲洗48h后尿红细胞计数及膀胱痉挛、尿管堵塞、漏尿、尿路感染发生情况,比较两组尿路感染者尿液培养细菌菌落数差异,并通过Kolcaba舒适状况量表(Kolcaba comfort status scale,GCQ)评分评估两组膀胱冲洗主观舒适度。结果研究组持续膀胱冲洗时间、冲洗液清亮时间、冲洗48h后尿红细胞计数及膀胱痉挛、尿管堵塞、漏尿、尿路感染发生率少(低)于对照组(P<0.05),而膀胱冲洗主观舒适度高于对照组(P<0.05)。研究组尿路感染者尿液培养检出大肠埃希菌、变形杆菌、克雷伯杆菌、粪链球菌及其他类细菌菌落数均低于对照组(P<0.05)。结论新型膀胱冲洗引流装置可减少PKRP术后膀胱冲洗患者相关并发症,减少尿路感染细菌菌落数,改善患者舒适度。 OBJECTIVE To investigate the effect of new bladder irrigation and drainage device on urinary tract infection and urine colony count in patients with continuous bladder irrigation.METHODS Retrospective analysis was performed on clinical data of 82patients undergoing plasmakinetic resection of prostate(PKRP)in Affiliated Hospital of Youjiang Medical College for Nationalities from Jul.2017to Jun.2019.Among them,39patients were treated with traditional bladder irrigation and drainage device for continuous bladder irrigation from Jul.2017to Jun.2018(control group),and 43patients underwent continuous bladder irrigation through new bladder irrigation drainage device from Jul.2018to Jun.2019(observation group).The continuous bladder irrigation time,irrigation solution clearing time,urinary red blood cell count and occurrence of bladder spasm,urinary catheter obstruction,urine leakage and urinary tract infection after 48hof irrigation were recorded in the two groups.The bacterial colony count in urine culture was compared between the two groups of patients with urinary tract infection.The subjective comfort of bladder irrigation was evaluated by Kolcaba General Comfort Questionnaire(GCQ)score.RESULTS The continuous bladder irrigation time,irrigation solution clearing time and urinary red blood cell count and incidence rates of bladder spasm,urinary catheter obstruction,urine leakage and urinary tract infection in the observation group were significantly lower than that in the control group(P<0.05),and the subjective comfort of bladder irrigation was significantly higher than that in the control group(P<0.05).The bacterial colony count of Escherichia coli,Proteusbacillus vulgaris,Klebsiella pneumonia and Streptococcus faecalis detected in the urine culture in patients of the observation group were significently lower than that in the control group(P<0.05).CONCLUSION New bladder irrigation and drainage device can reduce the incidence rates of related complications,reduce the bacterial colony count of urinary tract infection and improve patient comfort.
