经脐单孔腹腔镜切除术对急性胆囊炎患者术后胃肠功能恢复及预后的影响

Effect of Transumbilical Single Port Laparoscopic Cholecystectomy on Postoperative Gastrointestinal Function Recovery and Prognosis in Patients with Acute Cholecystitis

目的探讨经脐单孔腹腔镜切除术对急性胆囊炎患者术后胃肠功能恢复及预后的影响。方法将2020年1月~2019年1月收治的80例急性胆囊炎患者为例,根据治疗方式不同,随机分为两组,即观察组和对照组,每组患者均40例。其中,对照组患者采用传统开腹胆囊切除术治疗;观察组患者经脐单孔腹腔镜切除术治疗,观察与比较两组患者临床治疗效果。结果经研究发现(1)观察组患者手术时间(67.1±7.8)、术中出血量(21.2±6.3)及胃肠蠕动恢复时间(27.9±2.5)均短于对照组,并且差异明显(P<0.05),具有统计学意义。(2)观察组患者术后1 d、3 d、7 d的VAS评分均低于对照组,并且差异明显(P<0.05),具有统计学意义。(3)观察组患者术后并发症发生率5%(2/40)要低于对照组25%(10/40),且差异比较明显(P<0.05),具有统计学意义。结论对急性胆囊炎患者,采用经脐单孔腹腔镜切除术治疗治疗,创伤比较小,降低了患者并发症发生率,有利于患者术后预后。

Objective To investigate the effect of transumbilical single port laparoscopic resection on gastrointestinal function recovery and prognosis in patients with acute cholecystitis.Methods 80 cases of acute cholecystitis in our hospital from January 2020 to January 2019 were randomly divided into observation group and control group,with 40 cases in each group.Among them,the control group was treated with traditional open cholecystectomy;the observation group was treated with transumbilical single port laparoscopic resection,and the clinical therapeutic effects of the two groups were observed and compared.Results The study found that(1)the operation time(67.1±7.8),intraoperative blood loss(21.2±6.3)and gastrointestinal peristalsis recovery time(27.9±2.5)in the observation group were shorter than those in the control group,and the difference was significant(P<0.05).(2)The VAS scores of the observation group at 1 D,3 D and 7 d after operation were lower than those of the control group,and the difference was significant(P<0.05),with statistical significance.(3)The incidence of postoperative complications in the observation group was 5%(2/40)lower than that in the control group(25%(10/40),and the difference was statistically significant(P<0.05).Conclusions For patients with acute cholecystitis,transumbilical single port laparoscopic resection treatment,less trauma,reduce the incidence of complications,is conducive to the prognosis of patients,worthy of clinical application and promotion.
