血清CTRP-3、Hcy对脑梗死患者发生脑出血转化的预测作用 被引量:1

摘要 目的 分析血清补体C1q肿瘤坏死因子相关蛋白-3(CTRP-3)、同型半胱氨酸(Hcy)对脑梗死患者发生脑出血转化(HT)的预测作用.方法 选取2017-05—2019-10间郑州大学第一附属医院收治的158例经静脉溶栓治疗的脑梗死患者,24 h后根据MRI检查结果将其分为HT组(28例)、非HT组(130例).比较HT组、非HT组血清CTRP-3、Hcy水平;采用Logistic回归分析血清CTRP-3、Hcy是否为脑梗死后HT的危险因素;采用受试者工作特征曲线(ROC)分析血清CTRP-3、Hcy对脑梗死后HT的预测价值.结果 HT组血清CTRP-3水平低于非HT组,血清Hcy水平高于非HT组,差异均有统计学意义(P<0.05).经Logistic回归分析,血清CTRP-3、Hcy是脑梗死后HT的危险因素(P<0.05).ROC结果显示,血清CTRP-3、Hcy预测脑梗死后HT的曲线下面积(AUC)分别为0.862、0.827.结论 血清CTRP-3、Hcy对脑梗死后HT具有较高预测价值,可作为临床参考指标. Objective To analyze the predictive effect of serum complement C1q tumor necrosis factor related proteins-3(CTRP-3)and homocysteine(Hcy)on cerebral hemorrhage transformation(HT)in patients with cerebral infarction.Methods Selected 158 cases of cerebral infarction treated by intravenous thrombolysis in the First Affiliated Hospital of Zhengzhou University, after 24 h, they were divided into HT group(28 cases)and non-HT group(130 cases)according to MRI results. The serum CTRP-3 and Hcy levels in HT group and non-HT group were compared. Logistic regression was used to analyze whether serum CTRP-3 and Hcy were risk factors for HT after cerebral infarction. The predictive value of serum CTRP-3 and Hcy on HT after cerebral infarction was analyzed by ROC.Results The serum CTRP-3 level of HT group was lower than that of non-HT group(P<0.05), and the serum Hcy level was higher than that of non-HT group(P<0.05). Logistic regression analysis showed that serum CTRP-3 and Hcy were risk factors for HT after cerebral infarction(P<0.05). ROC results showed that the area under the curve(AUC)of HT after cerebral infarction predicted by serum CTRP-3 and Hcy was 0.862 and 0.827 respectively.Conclusion Serum CTRP-3 and Hcy have high predictive value for HT after cerebral infarction and they can be used as clinical reference indexes.
