手法复位小夹板联合石膏外固定与传统石膏外固定治疗老年桡骨远端骨折临床对比研究

Effects of manual reduction small-splint combined with plaster external fixation on wrist function and prognosis of elderly patients with distal radius fractures

目的 探讨手法复位小夹板联合石膏外固定与传统石膏外固定治疗老年桡骨远端骨折的临床价值.方法 前瞻性纳入2018-03—2019-05间于济源市中医院骨科行手法复位外固定治疗的78例老年桡骨远端骨折患者,依据外固定方法分为传统石膏外固定组(石膏外固定组)和小夹板联合石膏外固定组(联合小夹板组),各39例.比较2组患者的基线资料和随访12个月期间的治疗有效率、腕关节功能,以及预后效果.结果 联合小夹板组的治疗有效率、腕关节功能评分均高于石膏外固定组,桡骨缩短长度、外生骨痂出现时间、骨折线消失时间均短于石膏外固定组,差异均有统计学意义(P<0.05).结论 手法复位小夹板联合石膏外固定治疗老年桡骨远端骨折,效果显著,腕关节功能理想,有利于缩短愈合时间和改善预后.但仍需要更大样本量的前瞻性随机对照研究予以证实.

Objective To explore effects of manual reduction small-splint combined with plaster external fixation on wrist function and prognosis of elderly patients with distal radius fractures(DRF).Methods Prospective inclusion of 78 elderly patients with distal radius fracture treated with manual reduction and external fixation between 2018-03 and 2019-05 in the Department of Orthopaedics of Jiyuan traditional Chinese Medicine Hospital, According to the external fixation method, they were divided into traditional gypsum external fixation group(gypsum external fixation group)and small splint combined with gypsum external fixation group(combined with small splint group),39 cases each. the baseline data and the effective rate of treatment, wrist function, and prognostic effect during the 12-month follow-up period were compared.Results The effective rate and wrist function score of combined splint group were higher than those of plaster external fixation group. The shortening length of radius, the time of appearance of external callus and the time of disappearance of fracture line were shorter than those of plaster external fixation group. All the differences were statistically significant(P<0.05).Conclusion Manual reduction of small splint combined with plaster external fixation in the treatment of elderly distal radius fracture, the effect is remarkable, wrist function is ideal, it is helpful to shorten the healing time and improve the prognosis. But a larger sample size, a prospective randomized controlled study, is needed.
