老年重症颅脑损伤患者术后颅内感染的危险因素及防护对策

Risk factors and protective strategies of postoperative intracranial infection in aged patients with severe craniocerebral injury

目的探讨老年重症颅脑损伤患者手术后颅内感染发生的危险因素,并就其主要因素给予相应的防护对策。方法纳入2018年2月至2020年2月本院收治的重症颅脑损伤患者50例,收集患者基础资料和临床信息,将出现术后颅内感染的患者设为发生组,未感染患者设为未发生组。采用单因素分析和多因素logistic回归法分析患者术后发生颅内感染的相关因素和主要因素。结果纳入的50例患者中,7例发生颅内感染,发生率为14.00%。经多因素logistic回归分析,手术时间、低蛋白血症及引流管留置时间等均为导致老年重症颅脑损伤术后颅内感染的独立危险因素。结论手术时间、低蛋白血症及引流管留置时间等均为老年重症颅内损伤患者术后出现颅内感染的独立危险因素,临床应提高护理人员的综合素质和能力,根据上述因素,制定相应防护对策,从而减少颅内感染的发生,促进患者快速康复。

Objective To explore the risk factors of intracranial infection after operation in elderly patients with severe craniocerebral injury,and to give corresponding protective measures on the main factors.Methods 50 patients with severe craniocerebral injury who were operated in our hospital from February,2018 to February,2020 were included.Their basic data and clinical information were collected.The patients with postoperative intracranial infection were set as an occurrence group,and those without a non-infection group.Univariate analysis and multivariate logistic regression were used to analyze the related factors and main factors of postoperative intracranial infection.Results Among the 50 cases,7(14.00%)got intracranial infection.The results of multivariate logistic regression showed that operation time,hypoproteinemia,and indwelling time of drainage tube were the independent risk factors for postoperative intracranial infection.Conclusion Operation time,hypoproteinemia,and indwelling time of drainage tube are independent risk factors for intracranial infection in elderly patients with severe intracranial injury.The comprehensive quality and ability of nursing staff should be improved and appropriate protective measures formulated to reduce intracranial infection and promote their recovery.
