805株铜绿假单胞菌的临床分布与耐药性分析 被引量:1

摘要 目的分析本院805株铜绿假单胞菌的临床分布及耐药性特点,为临床合理使用抗生素及有效的院感控制提供依据。方法通过回顾性分析方法,采用WHONET5.6软件,对本院2018年6月至2019年12月检出的铜绿假单胞菌的标本来源、科室分布和耐药性等特征进行分析。结果2018年6月至2019年12月共检出铜绿假单胞菌805株,这些菌株大部分分布在烧伤整形外科24.60%(198/805),泌尿外科16.02%(129/805),创伤骨科10.31%(83/805)和重症医学科6.46%(52/805);标本分布:伤口分泌物379株,尿液192株,痰液97株,血液82株,脓液18株,其他标本37株;药敏结果显示,铜绿假单胞菌对多粘菌素B和阿米卡星的敏感性较高,敏感率分别为98.01%(789/805)和96.64%(778/805),而对氨曲南和替卡西林/棒酸的敏感性较低,敏感率分别为76.89%(619/805)和54.29%(437/805),对其他常见抗生素(天然耐药除外)的敏感率则在80.00%以上。结论本院铜绿假单胞菌的临床分布不均匀,部分药物的耐药性也比较高,医院相关部门要做好病原菌监测工作和合理使用抗生素。 Objective To analyze the clinical distribution and drug resistance characteristics of 805 strains of pseudomonas aeruginosa at our hospital,and to provide basis for the rational use of antibiotics and effective nosocomial infection control.Methods The sample source,department distribution,and drug resistance of pseudomonas aeruginosa detected in our hospital from June 2018 to December 2019,were retrospectively analyzed by WHONET5.6 software.Results A total of 805 strains of pseudomonas aeruginosa were detected from June,2018 to December,2019;most of which were distributed in orthopedic surgery for burns(24.60%,198/805),urology(16.02%,129/805),orthopedic trauma,(10.31%,83/805)and intensive care medicine(6.46%,52/805).Specimen distribution:379 strains of wound secretions,192 strains of urine,97 strains of sputum,82 strains of blood,18 strains of pus,and 37 other specimens.The drug sensitivity results showed that the sensitivity of pseudomonas aeruginosa to polymyxin B and amikacin were higher,with the sensitivities of 98.01%(789/805)and 96.64%(778/805),respectively;the sensitivities to aztreonam and ticarcillin clavulanic acid were lower,76.89%(619/805)and 54.29%(437/805),respectively;and the sensitivities to other common antibiotics(except natural resistance)were more than 80.00%.Conclusion The clinical distribution of pseudomonas aeruginosa at our hospital is not uniform,and the drug resistance of some drugs is relatively high.The relevant departments of hospitals should pathogen monitoring and rational use of antibiotics exactly.
