妇科手术患者应用静脉血栓栓塞症危险因素评估评分简表的护理价值

Nursing value of risk factor assessment of venous thromboembolism for patients undergoing gynecological surgery

目的观察妇科手术患者应用静脉血栓栓塞症危险因素评估评分简表的护理价值。方法选取本院2017年4月至2019年6月就诊的妇科手术患者62例,随机分为两组,各31例。对照组采用Caprini血栓风险评估表构建护理措施,观察组采用本院自制的妇科静脉血栓栓塞症危险因素评估评分简表构建护理措施;比较两组患者应用不同评价方式的分级情况、术后筛查VTE时间、术后发生静脉血栓的发生率及严重程度。结果两组患者筛查结果比较,差异无统计学意义(P>0.05);观察组筛查操作时间、术后7 d的Caprini评分表分别为(5.23±2.34)min、[1(1,3)]分,均显著低于对照组(10.52±5.72)min、[2(2,7)]分,差异均有统计学意义(均P<0.05);观察组术后7 d的VTE发生率为3.23%,显著低于对照组25.81%,差异有统计学意义(P<0.05)。结论妇科手术患者采用静脉血栓栓塞危险因素评估评分简表,可以简化VTE评估时间,降低术后发生VTE风险,减轻VTE的严重程度,且与Caprini血栓风险评估表的筛查结果准确性相一致,可将评估提前至术前,有助于更早期实施VTE分级护理措施。

Objective To observe the nursing value of risk factor assessment of venous thromboembolism for patients undergoing gynecological surgery.Methods 62 patients who underwent gynecological surgery at our hospital from April,2017 to June,2019 were randomly divided into a control group and an observation group,with 3 cases in each group.The control group used Caprini thrombotic risk assessment form to construct predictive nursing measures.In the observation group,predictive nursing measures were constructed by using a simple scale of risk factor assessment for gynecological venous thromboembolism.The grades,incidence,and severity of venous thrombosis in the two groups were compared.Results There was no statistical difference in the screening results between the two groups(P>0.05).The screening operation time and Caprini score 7 days after the operation were significantly lower in the observation group than in the control group[(5.23±2.34)min vs.(10.52±5.72)min and 1(1,3)vs.2(2,7);both P<0.05].The incidence of VTE was lower in the observation group than in the control group(3.23%vs.25.81%;P<0.05).Conclusion The use of the venous thromboembolism risk factor assessment score table in gynecological surgery patients can shorten the VTE assessment time and reduce the risk of postoperative VTE occurrence and the severity of VTE,and is consistent with the accuracy of the screening results of the Caprini thrombus risk assessment form,which can advance the assessment to before operation and facilitate earlier implementation of VTE grading care measures.
