早期功能锻炼对手部深度烧伤患者功能恢复和外观改善的影响

Effect of Early Functional Exercise on Functional Recovery and Appearance Improvement of Patients with Hand Deep Burn

目的:研究早期功能锻炼对手部深度烧伤患者功能恢复和外观改善的影响。方法:回顾性分析笔者医院2016年3月-2019年1月收治的95例手部深度烧伤患者的临床资料,所有患者依据入院先后顺序分为对照组(n=46)和观察组(n=49),均接受削痂植皮术治疗。对照组在此基础上联合常规康复干预,观察组在对照组基础上联合早期功能锻炼,比较两组手部瘢痕评分、手功能评分、手关节活动度、手部外观及并发症发生情况。结果:观察组血管分布、色泽、厚度及柔软度评分均低于对照组,差异有统计学意义(P<0.05)。干预后,两组手功能评分均上升,且观察组高于对照组,差异有统计学意义(P<0.05)。观察组关节活动度优良率及手部外观优良率均高于对照组,差异有统计学意义(P<0.05)。两组植皮均成功,无再次植皮患者,且无感染、创面愈合不良等并发症发生。结论:早期功能锻炼能够降低手部深度烧伤患者的瘢痕严重程度,促进手部功能恢复,并有利于手部外形的改善。

Objective To study the effect of early functional exercise on functional recovery and appearance improvement of patients with hand deep burns.Methods The clinical data of 95 patients with deep hand burns admitted to our hospital from March 2016 to January 2019 were retrospectively analyzed.All patients were divided into the control group(n=46)and the observation group(n=49)according to the admission order,and all received escharectomy and skin grafting.The control group was combined with routine rehabilitation intervention on this basis.The observation group was combined with early functional exercise on the basis of the control group.The hand scar score,hand function score,hand joint activity,hand appearance and complications were compared between the two groups.Results The vascular distribution,color,thickness and softness scores of the observation group were lower than those of the control group,the differences were statistically significant(P<0.05).After the intervention,the hand function scores of the two groups increased,and the observation group was higher than the control group(P<0.05).The excellent and good rate of joint motion and hand appearance in the observation group were higher than those in the control group(P<0.05).Skin grafting was successful in both groups.There was no skin graft again,and no infection,wound healing and other complications occurred.Conclusion Early functional exercise can reduce the scar severity of patients with deep hand burns,promote the recovery of hand function,and is benefi cial to the improvement of hand shape.
