血管靶向光动力疗法与血卟啉单甲醚光动力疗法治疗微静脉畸形临床对比研究

Clinical Comparative Study between Vascular Targeted Photodynamic Therapy and Hematoporphyrin Monomethyl Ether Photodynamic Therapy in the Treatment of Patients with Microvenous Malformation

目的:比较血管靶向光动力疗法(V-PDT)与血卟啉单甲醚光动力疗法(HMME-PDT)治疗微静脉畸形(原称鲜红斑痣)的临床疗效。方法:选取2017年6月-2018年11月在笔者医院皮肤科就诊的42例微静脉畸形患者,根据治疗方法不同将所有患者分为V-PDT组(20例)和HMME-PDT组(22例),V-PDT组患者给予V-PDT治疗,HMME-PDT组患者给予HMME-PDT治疗。观察并比较两组的有效率、不良反应发生情况以及疼痛评分。结果:HMME-PDT组有效率95.45%,显著高于V-PDT组的70.00%,两组有效率比较,差异具有统计学意义(P<0.05)。HMME-PDT组不良反应发生率9.09%,显著低于V-PDT组的40.00%,且差异具有统计学意义(P<0.05)。HMME-PDT组手术疼痛评分显著低于V-PDT组,且差异具有统计学意义(P<0.05)。结论:在微静脉畸形患者的治疗过程中,采用HMME-PDT进行治疗,临床疗效更加显著,可显著控制不良反应发生率,值得临床推广应用。

Objective To compare the clinical efficacy between vascular targeted photodynamic therapy(V-PDT)and hematoporphyrin monomethyl photodynamic therapy(HMME-PDT)in the treatment of patients with port wine stain(PWS).Methods 42 patients with PWS were selected from June 2017 to November 2018.According to different treatment methods,all patients were divided into the V-PDT group(20 cases)and the HMME-PDT group(22 cases).The V-PDT group was treated with V-PDT,and the HMME-PDT group was treated with HMME-PDT.The efficacy,adverse reactions and scores of pain in the two groups were observed and compared.Results The effective rate in the HMME-PDT group(95.45%)was signifi cantly higher than that in the V-PDT group(70.00%),with statistical difference(P<0.05).The incidence of adverse reactions in the HMME-PDT group(9.09%)was significantly lower than that in the V-PDT group(40.00%)and with statistical difference(P<0.05).The score of pain in the HMME-PDT group was significantly lower than that in the V-PDT group(P<0.05).Conclusion HMME-PDT is more effective in the treatment of patients with PWS.It can signifi cantly control the incidence of adverse reactions and is worthy of clinical promotion and application.
