益生菌片含服对不同程度慢性牙周炎患者牙周PLI、BI、PD、AL水平及美学修复的影响

Effects of Probiotics Tablets on Periodontal PLI,BI,PD,AL Levels and Aesthetic Repair in Patients with Different Degrees of Chronic Periodontitis

目的:探究益生菌片含服对不同程度慢性牙周炎患者牙周菌斑指数(PLI)、探诊出血指数(BI)、牙周袋深度(PD)、附着丧失(AL)水平及美学修复的影响。方法:选取沧州市人民医院牙周慢性牙周炎120例患者为研究对象,根据患者病情程度参照Armitage推荐标准,将患者分度:轻度(0.6<AL<2.0mm)40例、中度(2.0≤AL<2.5mm)40例、重度(AL≥2.5)40例,并将轻、中、重度患者随机分到对照组和观察组中。对照组:行常规基础治疗洁治、刮治和根面平整;观察组:在对照组治疗的基础上每日含服益生菌片。所有患者在接受治疗前1周及治疗后6周、3个月均完成牙周临床检查及唾液样本收集。比较两组轻、中、重度牙周炎患者治疗前、治疗后6周、3个月牙周PLI、BI、PD、AL水平及唾液中牙龈卟啉单胞菌(Porphyromonas gingivalis,Pg)和具核梭杆菌(Fusobacterium nucleatum,Fn)水平,并对两组患者治疗前后牙周美学指标龈乳头指数(PI)和龈乳头高度(PH)进行对比。结果:治疗后,两组患者PLI、BI、PD、AL均呈下降趋势,治疗3个月后PLI、BI、PD、AL水平均明显低于治疗前,且观察组明显低于对照组,组间比较差异均有统计学意义(P<0.05)。治疗后6周,Pg、Fn检出率均下降;治疗后3个月,Pg、Fn检出率上浮;且治疗后3个月观察组患者Fn检出率明显低于对照组,比较差异有统计学意义(χ2=4.302,P=0.038)。治疗后,两组患者PI和PH均呈上升趋势,治疗后6周、治疗后3个月PI和PH水平均明显高于治疗前,且观察组明显高于对照组,组间比较差异均有统计学意义(P<0.05)。结论:益生菌片含服联合常规基础治洁治、刮治和根面平整对不同程度慢性牙周炎临床疗效显著,能够有效改善患者牙周PLI、BI、PD、AL水平,并提高龈乳头指数及龈乳头高度,具有美学修复效果,值得临床推广应用。

Objective To investigate the effects of probiotic tablets on periodontal plaque index(PLI),probing bleeding index(BI),periodontal pocket depth(PD),attachment loss(AL)level and aesthetic restoration in patients with different degrees of chronic periodontitis.Methods The study selected 120 patients with chronic periodontitis in periodontal disease in Cangzhou People's Hospital.According to the patient's condition,the patient was classified according to the recommended criteria of Armitage.The patient was classifi ed into mild(0.6<AL<2.0mm)40 cases and moderate(2.0≤AL<2.5mm).There were 40 cases of severe cases(AL≥2.5)in 40 cases with AL<2.5mm,and mild,moderate and severe patients were randomly assigned to the control group and the observation group.The control group underwent conventional basic treatment for scaling,scaling and root planing,the observation group received probiotic tablets daily on the basis of the control group.All patients completed periodontal clinical examination and saliva sample collection 6 weeks and 3 months after treatment for 1 week before treatment.Comparing the two groups of patients with mild,moderate,and severe periodontitis before treatment,6 weeks and 3 months after treatment,PLI,BI,PD,AL levels and saliva),Porphyromonas gingivalis(Pg)And the level of Fusobacterium nucleatum(Fn),and the periodontal aesthetic index PI nipple index(PI)and licking nipple height(PH)were compared before and after treatment.Results After treatment,the PLI,BI,PD,and AL of the two groups showed a downward trend.After 3 months of treatment,the levels of PLI,BI,PD,and AL were signifi cantly lower than those before treatment,and the observation group was significantly lower than the control group.All were statistically significant(P<0.05).The detection rates of Pg and Fn decreased at 6 weeks after treatment,and the detection rates of Pg and Fn appeared up to 3 months after treatment.The detection rate of Fn in the observation group was signifi cantly lower than that in the control group at 3 months after treatment.The difference was statistically signifi cant(χ2=4.302,P=0.038).After treatment,both PI and PH showed an upward trend in the two groups.PI and PH levels were signifi cantly higher than those before treatment at 6 weeks after treatment and 3 months after treatment,and the observation group was signifi cantly higher than the control group.The differences were statistically significant(P<0.05).Conclusion Probiotics combined with conventional basic treatments for scaling,scaling and root planing have signifi cant clinical effects in treating different degrees of chronic periodontitis,which can effectively improve the periodontal PLI,BI,PD,AL levels and improve the nipple index.And the height of the nipple,with aesthetic restoration effect,is worthy of clinical application.
