摘要
目的探讨泛素样含PHD和环指域蛋白1(UHRF1)与室管膜瘤增殖的相关性及其预测肿瘤预后的临床价值。方法回顾性分析2005年1月至2017年4月接受室管膜瘤手术治疗195例患者的临床、病理和随访资料。运用COX回归模型、Pearson Chi-Square和Spearman’s correlation分析UHRF1与各临床病理指标的关系,采用Kaplan-Meier生存分析评估UHRF1表达与室管膜瘤患者预后的相关性。结果 UHRF1表达部位位于细胞核。195例室管膜瘤患者中,UHRF1阳性高表达36例,阳性低表达81例,阴性表达78例。统计学分析提示UHRF1表达水平与室管膜瘤组织学分级(P<0.05)和肿瘤发生部位(P<0.05)具有相关性;且UHRF1表达与Ki67增殖指数呈正相关(P<0.05),Pearson’s分析两者相关系数达0.449。此外,UHRF1高表达患者的总生存期及无病生存期均明显缩短。ROC分析提示UHRF1在室管膜瘤患者的预后分析中是重要的相关分子指标。结论 UHRF1与室管膜瘤的增殖密切相关,可作为判断室管膜瘤患者预后的生物学指标之一,协助临床制定更有效的室管膜瘤治疗和监测方案。
Aim To investigate the correlation between ubiquitin-like with PHD and ring finger domain 1(UHRF1) and ependymal tumor proliferation and its clinical value in predicting tumor prognosis. Methods The data of 195 patients with ependymal tumors who underwent resection at Huashan Hospital,Fudan University from January 2005 to April 2017 were analyzed retrospectively. The correlation between UHRF 1 and clinicopathologic characteristics were explored via COX regression model, Pearson Chi-Square and Spearman’s correlation. Kaplan-Meier method was used to clarify the correlation of UHRF1 with clinical pathological parameters. Results UHRF1 was expressed in the nucleus. Among the 195 ependymal tumors FFPE tissues, there were 36 cases with high UHRF1 expression, 81 cases with low UHRF1 expression and 78 cases with negative UHRF1 expression. Statistical analysis revealed the correlations of UHRF1 with histological grade(P<0.05) and localization(P<0.05). The expression of UHRF1 was closely related to the positive index of Ki67(P< 0.05). Pearson’s correlation coefficient between UHRF1 and Ki67 was 0.449. In addition, the overall survival and disease-free survival of patients with high expression of UHRF1 were significantly shortened. The ROC working curve showed that UHRF1 protein was an important molecular marker related to prognosis in patients with ependymal tumor. Conclusion UHRF1 is closely related to the proliferation of ependymal tumors, and can be used as an important index to judge the prognosis of ependymal tumors, which can help the clinical development of more effective treatment and monitoring programs for ependymal tumors.
