健康成年人能量摄入时间与胰岛素敏感性的相关性研究

摘要 目的研究健康志愿者能量摄入的时间是否与胰岛素敏感性具有相关性。方法52例18~50岁的健康志愿者连续戴10 d腕动监控器,并在第7天至第9天完成3 d的饮食日记。第11天受试者在20:00开始禁食不禁水,次日上午8:00开始进行5 h口服葡萄糖耐量试验(5 hOGTT)。根据饮食日记计算早晚摄入总能量的比例及摄入总能量达到25%、50%和75%时的时间,采用Matsuda指数和胰岛素敏感指数(ISI)评估胰岛素敏感性。采用多元线性回归模型评估能量摄入的时间与胰岛素敏感性的相关性。结果44例受试者完成了试验过程。调整后的回归模型结果显示,早晨能量摄入量每增加10%,Matsuda指数平均增加2.8个单位(P<0.05)。夜间能量摄入比例与胰岛素敏感性呈负相关,但差异无统计学意义(P>0.05)。每天摄入能量达到25%的时间段内,每增加1 h,Matsuda指数降低1.4个单位,ISI降低0.016个单位(P<0.05)。结论健康成人每天早晨及时足量地摄入能量与胰岛素敏感性具有相关性,这为探索基于时间的饮食干预提供了一定的理论依据。 Objective To investigate whether the timing of energy intake was associated with insulin sensitivity in healthy volunteers.Methods A total of 52 healthy volunteers aged 18 to 50 years wore wrist motion monitors for 10 consecutive days and completed a 3-day food diary from 7 to 9 days.On day 11,the subjects began fasting at 20:00,followed by 5-hour oral glucose tolerance test(5hOGTT)at 8:00 am.The proportion of total energy intake in the morning and evening and the time of total energy intake reaching 25%,50% and 75% were calculated from the food diary,and insulin sensitivity was assessed using the Matsuda index and Insulin sensitivity index(ISI).The correlation between the energy intake time and insulin sensitivity was evaluated using a multiple linear regression model.Results Forty-foursubjects completed the procedure.The results of the adjusted regression model showed that for every 10% increase in the morning energy intake,the Matsuda index increased by an average of 2.8 units(P<0.05).The proportion of the nocturnal energy intake was negatively correlated with insulin sensitivity,but not statistically significant(P>0.05).During the time period when the daily energy intake reached 25%,Matsuda index decreased by 1.4 units and ISI decreased by 0.016 units for every 1 hour increasing(P<0.05).Conclusion Adequate and timely intake of energy each morning in the healthy adults is associated with the insulin sensitivity,which providesa theoretical basis for exploringtime-based dietary interventions.
