平针法结合新型运动再学习康复治疗对缺血性脑卒中患者肢体运动功能及血清cAMP、cGMP的影响

Effect of flat acupuncture methods combined with new type of motor relearning rehabilitation on limb motor function and serum cAMP and cGMP in patients with ischemic stroke

目的研究平针法结合新型运动再学习康复疗法对缺血性脑卒中(CIS)患者肢体运动功能障碍的恢复,及血清cAMP、cGMP水平的影响。方法选择2018年8月-2019年8月在新疆医科大学第一附属医院针灸推拿科住院的90例确诊为CIS且肢体运动功能障碍的患者为研究对象,采用随机对照的研究方法分为治疗组(45例)和对照组(45例)。所有患者在试验前均经过结合患者具体病情给予控制血压、改善脑循环、稳定生命体征以及预防并发症等西医治疗。治疗组采用以平针法治疗;对照组常规针刺治疗。两组均结合新型运动再学习康复疗法治疗。针灸治疗每天治疗1次,10次为1个疗程;康复治疗5次为1疗程,每周治疗5次,治疗2个疗程后,测定患者血清cAMP、cGMP水平,采用CSS和FMA评分比较两组患者神经功能缺损程度以及其肢体运动功能恢复的程度。结果与对照组治疗后比较,治疗组患者治疗后FMA评分升高,CSS评分降低,血清cAMP水平升高,cGMP水平降低,差异有统计学意义(P<0.05)。与对照组比较,治疗组患者治疗后总有效率提高,差异有统计学意义(P<0.01)。结论平针法结合运动再学习康复疗法治疗CIS肢体运动功能障碍患者,疗效优于常规针刺法结合运动再学习康复疗法,方法值得临床推广应用。

Objective To study the effect of flat acupuncture combined with new type of motor relearning rehabilitation therapy on the recovery of limb motor dysfunction and serum camp and cGMP levels in the patients with cerebral ischemic stroke(CIS).Methods A total of 90 patients with CIS and limb motor dysfunction who were hospitalized in the Department of Acupuncture and Massage of the First Affiliated Hospital of Xinjiang Medical University from August 2018 to August 2019 were selected as the research objects.They were randomly divided into treatment group(45 cases)and control group(45 cases).All patients were given western medicine treatment such as controlling blood pressure,improving cerebral circulation,stabilizing vital signs and preventing complications.The treatment group was treated with flat needling,while the control group was treated with conventional acupuncture.Both groups were combined with new rehabilitation therapy of motor relearning.After two courses of the treatment,the serum levels of camp and cGMP were measured,and the degree of neurological impairment and recovery of limb motor function were compared between the two groups by CSS and FMA scores.Results Compared with the control group,the FMA score increased,the CSS score decreased,the serum cAMP level increased and the cGMP level decreased in the treatment group after the treatment with statistical significance(P<0.05).Compared with the control group,the total effective rate of the treatment group was improved with statistical significance(P<0.01).Conclusion The results show that the effect of the method of flat acupuncture combined with motor learning rehabilitation is better than that of the conventional acupuncture combined with motor learning rehabilitation therapy in the treatment of CIS limb motor dysfunction.
