1358例腹部手术患者术后肠胀气的疗效观察

Observation on the curative effect of 1358 case of abdominal operation patients with postoperative flatulence

目的研究通腑方对加速康复外科腹部手术患者术后恢复效果的影响。方法选取2016年1月-2018年12月在乌鲁木齐市友谊医院普外科接受腹部手术的1358例患者为研究对象,术后随机分为两组(对照组680例,治疗组678例),对照组患者采用常规治疗,治疗组患者在对照组基础上给予通腑方加速康复。分析比较两组患者术后第3天、第7天血常规(RBC、WBC、HB、L细胞计数、N细胞计数)和血生化指标(T-Bil、D-Bil、C-Bil、TP、ALB、ALT、AST、BUN、Cr、UA、Glu、钾、钙、钠、镁、氯、磷),治疗效果以及住院时间、术后首次肛门排气时间,首次排便时间。结果与对照组比较,治疗组患者术后第3天和第7天WBC、N细胞计数、UA水平均降低,TP、ALB、钙离子水平均升高,术后第7天氯离子水平升高,治疗有效率升高,患者住院时间、术后首次肛门排气时间和首次排便时间缩短,住院费用降低,差异均有统计学意义(P<0.05)。结论通腑方能够改善外科腹部手术后患者胃肠道功能,加速患者术后恢复,值得临床推广。

Objective To study the effect of Tongfu Prescription on postoperative rehabilitation of patients undergoing accelerated abdominal surgery.Methods A total of 1358 patients who had received abdominal surgery in the General Surgery Department of Urumqi Friendship Hospital from January 2016 to December 2018 were selected as the research objects,and were randomly divided into two groups after surgery(680 patients in control group and 678 patients in treatment group).Patients in the control group received routine treatment,while patients in the treatment group were administered Tongfu Prescription to accelerate rehabilitation after surgery on the basis of the control group.Compared the two groups of patients with routine blood cell count(RBC,WBC,HB,L,N cell count)and blood biochemical indicators(T-Bil,D-Bil,C-Bil,TP,ALB,ALT,AST,BUN,Cr and UA,Glu,potassium,calcium,sodium,magnesium,chlorine and phosphorus),treatment effect and the length of hospital stay,postoperative anal exhaust time and the first defecation time at postoperative day 3 and 7.Results Compared with the control group,in the treatment group the WBC,N cell count and UA level were decreased on the 3rd and 7th day after the operations,the TP,ALB,calcium level were increased,the chloride ion level was increased on the 7th day after the operations,the treatment efficiency was increased,the hospitalization time,the first anal exhaust time and the first defecation time were shortened,and the hospitalization cost was decreased(P<0.05).Conclusion Tongfu Prescription can improve the gastrointestinal function of patients after abdominal surgery,accelerate postoperative rehabilitation of patients,which is worthy of clinical promotion.
