血清蛋白酶原Ⅰ、胃泌素-17及PGR联合检测对早期胃癌的鉴别诊断价值

The value of combined detection of serum pepsionogenⅠ,gastrin-17 and PGR in the differential diagnosis of early gastric cancer

目的:分析血清蛋白酶原Ⅰ(PGⅠ)、胃泌素-17(G-17)及PGⅠ/PGⅡ比值(PGR)联合检测对早期胃癌的鉴别诊断价值。方法:选择2017年11月至2019年10月在我院治疗的79例早期胃癌患者为胃癌组,75例普通胃部疾病患者作为胃良性疾病组。比较2组血清PGⅠ、PGⅡ、G-17、PGR水平,以病理学或胃镜检查结果作为金标准,分析PGⅠ、G-17、PGR及三者联合诊断早期胃癌的敏感度、特异度及准确率。结果:胃癌组血清PGⅠ、PGR水平低于胃良性疾病组,G-17水平高于胃良性疾病组(P<0.01);2组血清PGⅡ水平比较,差异无统计学意义(P>0.05);PGⅠ+G-17+PGR联合诊断敏感度高于各指标单项检查结果,诊断准确率高于PGⅠ、G-17(P<0.05)。结论:PGⅠ、G-17、PGR均是鉴别早期胃癌的有效指标,联合检测能够提升早期胃癌诊断敏感度及准确率,提高胃癌检出率,为胃癌早期诊断以及治疗方案的制定提供参考。

Objective:To analyze the value of combined detection of serum pepsionogenⅠ(PGⅠ),gastrin-17(G-17)and PGⅠ/PGⅡratio(PGR)in the differential diagnosis of early gastric cancer.Methods:A total of 79 patients with early gastric cancer treated in our hospital from Nov 2017 to Oct 2019 were selected as the gastric cancer group,and 75 patients with general gastric disease were selected as the gastric benign disease group.The levels of serum PGⅠ,PGⅡ,G-17 and PGR between the two groups were compared.The pathology or gastroscopy results was used as the gold standard to analyze the sensitivity,specificity and accuracy of PGⅠ,G-17,PGR and the combined diagnosis of early gastric cancer.Results:The level of serum PGⅠand PGR was lower and the level of serum G-17 was higher in the gastric cancer group than those in the gastric benign disease group(P<0.01).The comparison of the level of serum PGⅡbetween the two groups was not statistically significant(P>0.05).The sensitivity of PGⅠ+G-17+PGR combined diagnosis was higher than the individual inspection results of each index,and the diagnostic accuracy rate was higher than PGⅠand G-17(P<0.05).Conclusions:PGⅠ,G-17 and PGR are effective indicators to identify early gastric cancer.Combined detection can improve the sensitivity and accuracy of early gastric cancer diagnosis,increase the detection rate of gastric cancer,and provide references for early gastric cancer diagnosis and treatment plan formulation.
