宝石能谱CT成像分析颈动脉粥样硬化斑块成分的应用价值研究 被引量:2

摘要 目的探究宝石能谱CT成像分析颈动脉粥样硬化斑块成分的应用价值。方法选取2017年12月至2018年12月在河南大学第一附属医院治疗的110例冠状动脉粥样硬化性心脏病颈动脉粥样硬化斑块患者。分别进行能谱CTA成像检查及超声检查,观察两种诊断方式的效果,以及分析斑块不同成分能谱曲线情况。结果超声检查显示的斑块检出率为96.02%,能谱CTA成像检查显示检出率为98.23%,但两种检查方式比较,差异无统计学意义(P>0.05)。110例患者中,有脂质斑块82处(36.28%),斑块内出血35处(15.49%),纤维斑块109处(48.23%)。纤维基质曲线斜率及有效原子序数高于斑块内出血和脂质成分,且斑块内出血高于脂质成分。越低的能量区,3种成分的吸收系数存在越大的差异,能谱曲线也会有越来越明显的离散程度。结论宝石能谱CT成像能够对颈动脉粥样硬化斑块成分进行有效分析,为斑块内出血、纤维基质、脂质成分提供量化数据。 【Objective】To explore the application value of gem spectroscopy CT imaging in the analysis of carotid atherosclerotic plaque components.【Methods】From December 2017 to December 2018,110 patients with carotid atherosclerotic plaque were selected,and performed CTA imaging examination and ultrasound examination to observe the effect of two diagnostic methods and analyze the compositional energy spectrum curve.【Results】The detection rate of plaque by ultrasound examination was 96.02%,and the detection rate of energy spectrum CTA imaging showed 98.23%.There was no significant difference between the two examination methods(P>0.05).Among the 110 patients,there were 82 lipid plaques(36.28%),35 plaque hemorrhages(15.49%),and 109 fibrous plaques(48.23%).The slope of the curve of the fibrous matrix,as well as the effective atomic number,is higher than the intra-plaque hemorrhage,and the intra-plaque hemorrhage is higher than the lipid component.The lower the energy zone,the greater the difference in the absorption coefficients of the three components,and the more obvious the dispersion of the energy spectrum curve.【Conclusion】Gem spectroscopy CT imaging can effectively analyze the components of carotid atherosclerotic plaque,and provide quantitative data for plaque hemorrhage,fibrous matrix and lipid components.
