自拟祛瘀通络方治疗气滞血瘀型心绞痛临床观察

Effect of Self-Made Quyu Tongluo Formula in Treatment of Qi Stagnation and Blood Stasis Syndrome of Angina

目的研究气滞血瘀型心绞痛使用自拟祛瘀通络方治疗效果。方法选择本院2018年5月—2019年5月收治慢性稳定型心绞痛92例,随机分为观察组和对照组各41例。入组患者均实施基础治疗,对照组口服复方丹参滴丸,观察组在此基础上使用自拟祛瘀通络方,均治疗4周。观察治疗后临床疗效,记录治疗前后全血黏度高切、全血黏度低切、血浆黏度值,统计治疗前后1周心绞痛发作次数、心绞痛发作持续时间及硝酸甘油用量。结果观察组治疗后总有效率为80.49%(33/41),高于对照组58.54%(24/41)(P <0.05);观察组治疗后全血黏度高切、全血黏度低切、血浆黏度值改善均优于对照组(P <0.05);观察组治疗后心绞痛发作频率、心绞痛持续时间、硝酸甘油用量均低于对照组(P <0.05)。结论自拟祛瘀通络方对气滞血瘀型心绞痛治疗效果确切,可有效改善心绞痛患者血液流变学指标及血管内皮功能,进而降低心绞痛发作频次及硝酸甘油用量。

Objective To study the effect of Self-made Quyu Tongluo Formula in treatment of Qi stagnation and blood stasis syndrome of angina. Methods Ninety-two patients with chronic stable angina pectoris from May 2018 to May 2019 in our hospital were dividedinto two groups randomly: the observation group and the control group,41 patients in each group. These patients were treated with the basic treatment. The control group received Compound Danshen Dripping Pills,and the observation group additionally received Self-made Quyu Tongluo Formula for 4 weeks. We observed the clinical effect after treatment,recorded thewholeblood high-shear viscosity,the whole blood low-shear viscosity,plasma viscosity value,nitric oxide( NO),serum endothelin( ET) and thromboxane B2( TXB2) before and after treatment,counted the frequency of angina pectoris attacks,the duration of angina pectoris attack and the dosage of nitorglycerin 1 week before and after treatment. Results The total effective rate of the observation group after treatment was 80. 49%( 33/41),which was higher than that of the control group( 58. 54%,24/41)( P <0. 05). After treatment,whole blood high-shear viscosity,whole blood low-shear viscosity and plasma viscosity value of the observation group were better than those of the control group( P < 0. 05). The frequency of angina pectoris attacks,the duration of angina pectoris attack and the dosage of nitorglycerin in the observation group were lower than those in the control group( P <0. 05). Conclusion Self-made Quyu Tongluo Formula was effective in the treatment of Qi stagnation and blood stasis syndrome of angina. It could effectively improve the blood rheology index and vascular endothelial function of patients with angina pectoris,and then reduce the frequency of angina pectoris and the dosage of nitroglycerin.
