多囊卵巢综合征患者红外热图与中医证型的关系研究 被引量:2

Relationship Between Infrared Thermal Imaging and Traditional Chinese Medicine Syndromes in Patients with Polycystic Ovary Syndrome

摘要 【目的】分析多囊卵巢综合征(PCOS)患者红外热图与中医证型的关系。【方法】选取2019年5月~2019年10月在广州中医药大学第一附属医院妇科门诊就诊,明确诊断为PCOS,并自愿到本院治未病科进行红外热成像检测的52例患者为研究对象。根据中医辨证分型将患者分为痰湿阻滞型、肝肾阴虚型、肝气郁结型、肾气虚弱型4型。用红外热像仪获取患者的红外热图,分析总结不同证型PCOS患者的红外热图特点。【结果】(1)中医证型分布:52例患者中,痰湿阻滞型10例,肝肾阴虚型11例,肝气郁结型21例,肾气虚弱型10例。(2)4种证型的躯干平均温度由高到低依次为肝气郁结型>肝肾阴虚型>痰湿阻滞型>肾气虚弱型,以肾气虚弱型温度最低,其温度明显低于肝肾阴虚型和肝气郁结型,差异有统计学意义(P<0.05)。(3)痰湿阻滞型患者中焦凉偏离最多,面部、手掌高热偏离,与躯体形成寒热对峙;肝肾阴虚型也是面部、手掌比躯干温度高,但温差不如痰湿阻滞型大;肝气郁结型多表现在上焦、中焦热偏离,面部与手掌温差大;肾气虚弱型患者表现为等温差热图。【结论】红外热成像技术能较为客观地反映不同证型PCOS患者的寒热虚实情况,并能精准定位,可为中医辨证提供客观化依据,并有助于促进中医诊疗的可视化、可量化和现代化。 Objective To analyze the relationship between infrared thermogram of the patients with polycystic ovary syndrome(PCOS)and traditional Chinese medicine(TCM)syndrome.Methods A total of 52 PCOS outpatients which were admitted to the gynecology department of the First Affiliated Hospital of Guangzhou University of Chinese Medicine and took the infrared thermal imaging examination voluntarily in the department of preventive treatment of diseases from May of 2019 to October of 2019 were included into the study.The PCOS patients were differentiated into 4 syndrome types,namely phlegm dampness obstruction syndrome,liver-kidney yin deficiency syndrome,liver-qi stagnation syndrome,and kidney qi insufficiency syndrome.The patients were photographed by infrared thermal imager,and the characteristics of the infrared thermogram in PCOS patients with different TCM syndromes were analyzed.Results(1)Of the 52 PCOS outpatients,10 cases were differentiated as phlegm dampness obstruction syndrome,11 cases were differentiated as liver-kidney yin deficiency syndrome,21 cases were differentiated as liver-qi stagnation syndrome,and 10 cases were differentiated as kidney qi insufficiency syndrome.(2)The average trunk temperature of the patients with liver-qi stagnation syndrome,liverkidney yin deficiency syndrome,phlegm dampness obstruction syndrome,and kidney qi insufficiency syndrome was in decreasing sequence,and the trunk temperature of the patients with kidney qi insufficiency syndrome was the lowest,obviously lower than that of the patients with liver-kidney yin deficiency syndrome and liver-qi stagnation syndrome,the difference being statistically significant(P<0.05).(3)Patients with phlegm dampness obstruction syndrome were characterized by the low temperature in middle energizer and high temperature in the face and palms,which formed the cold and heat confrontation in the body.The temperature in the face and palms of the patients with liver-kidney yin deficiency syndrome was also higher than that in the trunk,but the temperature difference was not as obvious as that of the patients with phlegm dampness obstruction syndrome.Patients with liver-qi stagnation syndrome were characterized by high temperature in the upper energizer and middle energizer,and with large temperature difference in the face and palms.Patients with kidney qi insufficiency syndrome showed an isothermal heat map.Conclusion Infrared thermal imaging technology not only objectively reflects the cold,heat,deficiency and excess nature of the syndromes of PCOS patients,but also is helpful for the accurate location of the affection,which can provide an objective basis for the TCM syndrome differentiation,and is beneficial to the visualization,quantization and modernization of TCM diagnosis and treatment.
