火针留刺激痛点联合经筋手法治疗膝骨关节炎的疗效观察

Curative Observation of Retaining of Heated Needle at Trigger Points Combined with Manipulation for Muscles along Meridians in Treating Knee Osteoarthritis

【目的】观察火针留刺激痛点联合经筋手法治疗膝骨关节炎的临床疗效。【方法】将42例膝骨关节炎(KOA)患者随机分为对照组与观察组。对照组给予经筋推拿手法治疗,每周治疗5次,连续治疗2周。观察组在对照组经筋手法治疗的基础上,给予激痛点火针针刺治疗,每2 d治疗1次,连续治疗2周。治疗2周后以及治疗4周随访时,观察2组患者治疗前后视觉模拟量表(VAS)评分、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)评分的变化情况,比较2组患者股四头肌肌张力的情况,并评价2组的安全性。【结果】(1)治疗后,2组患者的VAS评分和WOMAC总积分均明显改善(P<0.05),且观察组在改善VAS评分和WOMAC总积分方面均明显优于对照组,差异有统计学意义(P<0.05)。(2)治疗后,2组患者的股四头肌肌张力明显改善(P<0.05),且观察组在改善股四头肌肌张力方面均明显优于对照组,差异有统计学意义(P<0.05)。(3)本研究中,2组患者均未出现不良反应情况,无针刺及推拿不良事件的发生,安全性评级均为1级。【结论】火针留刺激痛点联合经筋手法治疗KOA,能明显改善患者的疼痛症状,提高患者的膝关节功能,疗效显著。

Objective To observe the curative efficacy of retaining of heated needle at trigger points combined with manipulation for muscles along meridians in treating knee osteoarthritis.Methods Forty-two knee osteoarthritis patients were randomly divided into control group and observation group,24 cases in each group.The control group received manipulation for muscles along meridians,5 times per week,the treatment lasting 2 continuous weeks.The observation group received retaining of heated needle at trigger points based on manipulation for muscles along meridians in the control group,one time every other day,the treatment lasting 2 continuous weeks.After 2 weeks of treatment and on follow-up after 4 weeks of treatment,the changes in scores of visual analogue scale(VAS)and Western Ontario and McMaster Universities osteoarthritis index(WOMAC)in the two groups before and after treatment,the muscular tension of quadriceps femoris was compared in the two groups,and the safety of the two groups were evaluated.Results(1)After treatment,the total scores of VAS and WOMAC in the two groups were improved(P<0.05),and the improvement in total scores of VAS and WOMAC of the observation group was superior to that of the control group(P<0.05).(2)After treatment,the muscular tension of quadriceps femoris in the two groups was improved(P<0.05),and the improvement in muscular tension of quadriceps femoris of the observation group was superior to that of the control group(P<0.05).(3)During the trial,no adverse reactions were seen in both groups,no acupuncture and massage adverse events occurred,achieving level one of security.Conclusion Heated needle at trigger points combined with manipulation for muscles along meridians is effective for improving pain symptom and knee joint functions in knee osteoarthritis patients.
