下肢机器人辅助步态训练对脑性瘫痪患儿康复效果及生活质量的影响研究 被引量:9

摘要 目的:评估下肢机器人辅助步态训练对脑性瘫痪(脑瘫)患儿康复效果及生活质量的影响。方法:选取82例痉挛型脑瘫患儿,按照随机数表法将其分为观察组和对照组,每组41例。对照组患儿采用常规康复训练,观察组患儿在常规康复训练基础上联合使用下肢机器人辅助步态训练。比较两组患儿康复效果,采用改良Ashworth痉挛分级量表(MAS)、Fugl-Meyer运动功能评定量表(FMA)、Berg平衡功能量表(BBS)、Barthel生活质量改良指数(MBI)以及儿科生活质量调查表(PedsQLTM),记录两组患儿治疗前、治疗3个月后的肌张力。结果:治疗3个月后,观察组患儿临床总有效率明显高于对照组,差异有统计学意义(x^2=5.145,P<0.05);治疗3个月后,两组患儿MAS评分均明显低于治疗前,且观察组明显低于对照组,差异有统计学意义(t=2.023,P<0.05);两组患儿FMA、MBI、BBS及PedsQLTM评分均明显高于治疗前,且观察组高于对照组,两组比较差异有统计学意义(t=2.337,t=2.134,t=2.381,t=2.335;P<0.05)。结论:下肢机器人辅助步态训练对脑瘫患儿应用效果较好,能有效改善患儿肌张力、肢体运动功能及平衡能力,提高患儿生活质量。 Objective:To explore the effects of lower limb robot-assisted gait training on rehabilitation effects and quality of life of children patients with cerebral palsy.Methods:82 children patients with spastic cerebral palsy were selected and they were divided into observation group and control group according to the random number table method,with 41 cases in each group.The children patients in control group received conventional rehabilitation training while the children patients in observation group received the combined method of lower limb robot-assisted gait training and conventional rehabilitation training.The rehabilitation effects of the two groups were compared,and the Modified Ashworth Spasm Scale(MAS),Fugl-Meyer Motor Function Assessment Scale(FMA),Berg Balance Scale(BBS),Barthel quality of life Modified Barthel Index(MBI)and Pediatric Quality of Life Questionnaire(PedsQLTM)were adopted to record the muscular tension of children patients in two groups before treatment and 3 months after treatment.Results:After 3 months of treatment,the total clinical effective rate of observation group was significantly higher than that of control group(x^2=5.145,P<0.05).The MAS scores of two groups after 3 months of treatment were significantly lower than those before treatment,and the score of observation group was significantly lower than that of control group(t=2.023,P<0.05).The scores of FMA,MBI,BBS and PedsQLTM of two groups after treatment were significantly higher than those before treatment,and the scores of observation group were significantly higher than those of control group(t=2.337,t=2.134,t=2.381,t=2.335,P<0.05),respectively.Conclusion:Lower limb robot-assisted gait training has a better application effect on children patients with cerebral palsy,and it can effectively improve the muscle tension,limb motor function and balance ability of children patients,and enhance the quality of life of them.
