牙周牙髓联合治疗与单纯牙周治疗对重度牙周炎治疗的效果和不良反应分析

Effect of Periodontal Pulp Combined Therapy and Simple Periodontal Therapy on Severe Periodontitis and Adverse Reactions

目的探讨牙周牙髓联合治疗与单纯牙周治疗对重度牙周炎治疗的疗效和不良反应。方法选取2014年1月至2019年12月扬州大学附属医院收诊的60例重度牙周炎患者作为研究对象,按照随机数字表法分为对照组和观察组,各30例。对照组采取单纯牙周治疗,观察组实施牙周牙髓联合治疗,比较两组患者的治疗总有效率、牙周状况指标、炎症因子指标、咀嚼效率、不良反应发生率。结果观察组患者总有效率高于对照组(96.67%vs.80.00%,P<0.05),治疗后,观察组患者菌斑指数、软垢指数、龈沟出血指数、牙周探诊深度均低于对照组(P<0.05),观察组患者C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)均低于对照组(P<0.05),观察组患者咀嚼效率高于对照组(P<0.05),观察组患者不良反应发生率6.67%与对照组不良反应发生率3.33%比较,差异无统计学意义(P>0.05)。结论牙周牙髓联合治疗对重度牙周炎的疗效优于单纯牙周治疗,可更加有效地改善患者牙周状况,减轻牙周炎症,促使咀嚼功能恢复,且不良反应少。

Objective To investigate the efficacy and adverse reactions of periodontal pulp combined therapy and simple periodontal therapy in the treatment of severe periodontitis.Methods From January 2014 to December 2019,60 patients with severe periodontitis in the department of stomatology,Affiliated Hospital of Yangzhou University were selected as the research objects and randomly divided into two groups,30 cases in each group.The control group was treated with simple periodontal treatment,and the observation group was treated with periodontal and endodontic treatment.The total effective rate,periodontal status index,inflammatory factor index,masticatory efficiency and incidence of adverse reactions were compared between the two groups.Results The total effective rate in the observation group was higher than that in the control group(96.67%vs.80.00%,P<0.05).After treatment,the plaque index,soft scale index,gingival sulcus bleeding index and periodontal probing depth in the observation group were lower than those in the control group(P<0.05),CRP,TNF-αand IL-6 in the observation group were lower than those in the control group(P<0.05),and the masticatory efficiency of the observation group was higher than that of the control group(P<0.05).The incidence of adverse reactions was 6.67%in the observation group and 3.33%in the control group,with no significant difference(P>0.05).Conclusion The curative effect of combined periodontal and endodontic therapy on severe periodontitis is better than that of periodontal treatment alone.It can more effectively improve the periodontal status of patients,reduce periodontitis,promote the recovery of masticatory function,and have fewer adverse reactions.
