彩色多普勒超声检测胎儿脐动脉及大脑中动脉血流频谱对预测宫内窘迫的临床价值分析 被引量:31

摘要 目的:分析彩色多普勒超声检测胎儿脐动脉、大脑中动脉血流频谱在预测宫内窘迫中的临床价值。方法:回顾性分析2017年6月至2018年6月前往我院进行产检的200例孕妇临床资料,依据是否发生宫内窘迫分为正常组160例及宫内窘迫组40例,对两组脐动脉、大脑中动脉血流频谱进行对比,并利用Pearson相关性分析方法对胎儿脐动脉、大脑中动脉血流频谱与宫内窘迫的相关性进行分析。结果:宫内窘迫组脐动脉血流频谱参数高于正常组,大脑中动脉血流频谱参数低于正常组,差异均有统计学意义(P<0.05);Pearson相关性分析结果提示,胎儿脐动脉血流频谱与宫内窘迫呈正相关性(r=0.841,P<0.05),大脑中动脉血流频谱与宫内窘迫呈负相关性(r=-0.777,P<0.05)。结论:胎儿脐动脉及大脑中动脉血流频谱与宫内窘迫的发生存在着密切的关联性,利用彩色多普勒超声进行检测有助于早期预测和早期干预,值得推广使用。 Objective:To analyze the clinical value of color Doppler ultrasonography in detecting fetal umbilical artery and middle cerebral artery blood flow spectrum in predicting intrauterine distress.Methods:A retrospective analysis of 200 cases of pregnant women who went to our hospital for medical examination from June 2017 to June 2018,according to whether or not intrauterine distress occurred,was divided into normal group 160 cases and intrauterine distress group 40 cases.The blood flow spectra of arteries and middle cerebral arteries were compared,and the correlation between fetal umbilical artery and middle cerebral artery blood flow spectrum in intrauterine distress was analyzed by Pearson correlation analysis.Results:The umbilical arterial blood flow spectrum parameters of the intrauterine distress group were higher than those of the normal group.The blood flow spectrum parameters of the middle cerebral artery were lower than the normal group,and the difference was statistically significant(P<0.05).The Pearson correlation analysis showed that there was a positive correlation between arterial blood flow spectrum and intrauterine distress(r=0.841,P<0.05).The middle cerebral artery blood flow spectrum was negatively correlated with intrauterine distress(r=-0.777,P<0.05).Conclusion:There is a close correlation between fetal umbilical artery and middle cerebral artery blood flow spectrum and intrauterine distress.The use of color Doppler ultrasound for early detection and early intervention is worthy of popularization.
