滋阴降火方治疗阴虚火旺型更年期综合征的临床评价及作用机制分析

Clinical Evaluation and Mechanism Analysis of Ziyin Jianghuo Decoction in the Treatment of Climacteric Syndrome of Yin Deficiency and Hyperactivity of Fire

目的研究针对阴虚火旺型更年期综合征病人采取滋阴降火方治疗的价值。方法对张家港市中医医院于2017年1月至2018年6月纳入的90例阴虚火旺型更年期综合征患者开展回顾性研究,依据治疗方法不同分成两组,每组45例,对照组予以补佳乐(戊酸雌二醇片)+黄体酮胶囊常规治疗,观察组在对照组基础上加用滋阴降火方进行治疗。评估两组患者治疗总有效率,比较两组患者促卵泡激素(FSH)水平升高、雌二醇(E2)、促黄体生成激素(LH)水平;对比两组的汉密顿焦虑量表(HAMA)和睡眠质量指数量表(PSQI)评分。结果观察组治疗总有效率为93.33%,与对照组的82.22%相比,差异有统计学意义(P<0.05);治疗后观察组患者FSH、E2及LH等指标均优于对照组(P<0.05);治疗后观察组患者HAMA及PSQI评分均显著低于对照组(P<0.05)。结论滋阴降火方在阴虚火旺型更年期综合征治疗中合理应用,疗效显著,患者激素水平、心理状态及睡眠质量均得到改善。

Objective The main content of this study is the value of Ziyin Jianghuo Decoction in the treatment of menopausal syndrome of yin deficiency and fire hyperactivity type.Methods From January 2017 to June 2018,90 patients with climacteric syndrome of yin deficiency and fire hyperactivity in Zhangjiagang Hospital of Traditional Chinese Medicine were enrolled in the study.According to the treatment,45 cases were treated with bujiale(estradiol valerate tablets)+progesterone capsules(control group).The other 45 cases were treated with Ziyin Jianghuo Decoction on the basis of the control group(observation group).The total effective rate,follicle stimulating hormone(FSH),estradiol(E2)and luteinizing hormone(LH)levels,HAMA and PSQI scores were evaluated.Results After treatment,the total effective rate of the observation group was 93.33%,and that of the control group was 82.22%.Compared with the two groups,the treatment effect of the observation group was significant higher(P<0.05);after treatment,the indexes of FSH,E2 and LH in the observation group were significantly better than those in the control group(P<0.05);after treatment,the observation group of patients with HAMA and PSQI were significantly lower than those of the control group(P<0.05).Conclusion Ziyin Jianghuo Decoction is reasonable used in the treatment of menopausal syndrome of yin deficiency and fire hyperactivity type,and the clinical effect is significant.The hormone level,psychological state and sleep quality of patients can be significantly improved.
