类风湿关节炎腕关节病变X线表现和磁共振影像对照分析 被引量:3

摘要 目的探讨分析类风湿关节炎腕关节病变X线表现和磁共振影像状况。方法回顾分析我院2016年5月至2018年12月收治的类风湿关节炎腕关节病变患者60例,各患者均接受双腕MR、X线片影像检查,分析比较检查结果。结果60例患者,120个腕关节均接受X线片和MRI检查,X线片共检出骨质侵蚀病灶共54个,MRI共检查骨质侵蚀病灶共403个;MR影像中,检出骨质侵蚀最多位点为分别为三角骨58.7%、月骨51.2%,而X线片检查三角骨1.6%、月骨5.6%,数据差异有统计学意义(χ^2=99.8203、64.9854,P=0.0001、0.0001)。结论临床在诊断类风湿关节炎腕关节病变时,若采用X线片无法得到明确诊断,可考虑使用MRI方式进行检查,此方式在诊断早期类风湿关节炎腕关节骨质侵蚀等方面作用突出。 Objective To explore and analyze the X-ray manifestations and MR imaging status of rheumatoid arthritis wrist joint lesions.Methods A retrospective analysis was conducted on 60 patients with rheumatoid arthritis and carpal joint lesions admitted to our hospital from May 2016 to December 2018.All patients received dual-wrist MR and X-ray imaging examination,and the results were analyzed and compared.Results Of 60 patients,120 wrist joints were examined by X-ray and MRI,54 bone erosion lesions were detected by X-ray,and 403 bone erosion lesions were detected by MRI.In the MR image,the most frequent sites of bone erosion were found to be triangular bone 58.7%and lunate bone 51.2%,respectively,while the X-ray images showed significant differences in triangular bone 1.6%and lunate bone 5.6%(χ^2=99.8203,64.9854,P=0.0001,0.0001).Conclusion In the clinical diagnosis of rheumatoid arthritis in the wrist joint lesions,if X-ray can not get a clear diagnosis,we can consider the use of MRI for examination,which plays a prominent role in the diagnosis of early rheumatoid arthritis in the wrist joint bone erosion and other aspects.
