摘要
目的探讨CT静脉造影与彩色多普勒超声在布加综合征图像质量及诊断效能的差异。方法回顾性分析临床怀疑37例布加综合征患者,该37例患者均行CT静脉造影及彩色多普勒超声检查且结果均经数字减影血管造影验证,观察两者在布加综合征的影像表现,检查结果以数字减影血管造影为金标准,分别计算两者的敏感度,特异度,阳性预测值、阴性预测值及准确率,比较两者在布加综合征的诊断准确率及图像质量是否差异具有统计学意义。以数字减影血管造影为金标准,对CT静脉造影诊断布加综合征的结果进行一致性检验。结果CT静脉造影的诊断准确率及图像质量均高于彩色多普勒超声,其差异有统计学意义(P<0.05);CT静脉造影的敏感度,特异度,阳性预测值、阴性预测值及准确率分别为90%,88%,96%,70%,89%,均高于彩色多普勒超声相应数据。CT静脉造影诊断布加综合征的结果与数字减影血管造影诊断结果一致性较好(Kappa=0.725)。结论CT静脉造影在布加综合征的诊断效能及图像质量上明显优于彩色多普勒超声,值得临床推广使用,可作为诊断布加综合征的首选影像学检查方法。
Objective To investigate the difference of image quality and diagnostic efficacy between CT venography and color Doppler ultrasound in Budd-Chiari syndrome.Methods Thirty-seven patients with Budd-Chiari syndrome were analyzed retrospectively.All 37 patients were examined by CT venography and color Doppler ultrasound,and the results were verified by digital subtraction angiography.The imaging findings of Budd-Chiari syndrome were observed.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of the two were calculated by digital subtraction angiography.The diagnostic accuracy and image quality of Budd-Chiari syndrome were compared between the two groups(P<0.05).There is significant statistical significance.According to the gold standard of digital subtraction angiography,the results of CT venography in the diagnosis of Budd-Chiari syndrome were examined.Results The diagnostic accuracy and image quality of CT venography were significantly higher than those of color Doppler ultrasound(P<0.05).The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of CT venography were 90%,88%,96%,70%and 89%,respectively,which were significantly higher than those of color Doppler ultrasound.The results of CT venography in the diagnosis of Budd-Chiari syndrome were in good agreement with those of digital subtraction angiography(Kappa=0.725).Conclusion CT venography is superior to color Doppler ultrasound in the diagnosis and image quality of Budd-Chiari syndrome.It is worth popularizing in clinic and is the first choice for Budd-Chiari syndrome.
