腹腔镜下乙状结肠、直肠肿瘤手术吻合口瘘137例临床分析

Clinical Analysis of 137 Cases of Anastomotic Leakage Under Laparos copic Surgery for Sigmoid Colon and Rectal Tumors

目的对乙状结肠和直肠肿瘤患者使用腹腔镜手术进行治疗的疗效进行观察。方法对该次研究在2014年7月—2019年9月所选择的137例乙状结肠和直肠肿瘤患者使用腹腔镜手术进行治疗的相关临床资料进行回顾性分析,对患者手术成功的结果、具体的手术情况进行观察。结果 131例手术获成功,手术成功率为95.6%;6例中转开腹,所占比例为4.6%;成功手术中34例行结肠造瘘,所占比例为25.9%;97例行Ⅰ期吻合保留肛门,所占比例为74.1%;其中10例发生术后出现吻合口瘘现象。术后行Ⅰ期吻合患者吻合口瘘发生率为10.3%。结论使用腹腔镜手术对乙状结肠和直肠肿瘤手术进行治疗安全性较高,对患者身体造成的伤害较小,且具有比开腹根治术更好的短期效果。

Objective To explore the clinical experience of laparoscopic surgery for sigmoid and rectal tumors.Methods 137 cases of laparoscopic operation for sigmoid colon and rectum tumors were retrospectively analyzed from July 2014 to September 2019.Outcome measures:the outcome of successful operation and the specific operation conditions were observed.Results 131 cases were successful,the probability of success was 95.6%.6 cases were transferred to laparotomy,accounting for 4.6%.Among the successful operations,34 underwent colostomy,accounting for 25.9%.97 routineⅠstage anastomosis retain the anus,the proportion is 74.1%;anastomotic fistula occurred in 10 cases.Patients with postoperative lineⅠmatch the incidence of anastomotic fistula was 10.3%.Conclusion Laparoscopic operation of sigmoid colon and rectum is safe and feasible,with less trauma,quick recovery of patients,and better short-term effect than laparotomy.
