分析高血压脑出血患者应用立体定向血肿抽吸联合尿激酶的效果

Analysis of the Effect of Stereotactic Hematoma Aspiration Combined with Urokinase in Patients with Hypertensive Cerebral Hemorrhage

目的探讨立体定向血肿抽吸联合尿激酶在治疗高血压脑出血患者中的应用效果。方法选取该院2017年1月—2019年10月收治的40例高血压脑出血患者作为研究对象,按照随机数表法分为对照组20例(开颅血肿清除术);观察组20例(采用立体定向血肿抽吸联合尿激酶治疗)。对比两组患者的临床效果、神经功能缺损情况、并发症情况以及临床指标。结果治疗后观察组神经功能缺损情况评分为(11.3±1.3)分低于对照组的(24.6±2.4)分,差异有统计学意义(t=26.689,P<0.05)。观察组治疗有效率为95.00%高于对照组的60.00%(χ^2=7.025,P=0.008)。治疗后,观察组患者血肿旁水肿面积为(7.16±1.32)cm^2小于对照组的(13.72±1.45)cm^2、血肿消失时间为(3.28±1.12)d短于对照组的(9.39±3.46)d,差异有统计学意义(t=18.324、21.251,P<0.05)。观察组并发症发生率为5.00%低于对照组的35.00%(χ^2=3.906,P<0.05)。结论通过对高血压脑出血患者采用立体定向血肿抽吸联合尿激酶治疗,能够有效减少其神经功能损伤,减少血肿旁水肿面积,术后并发症少,能够更好地促进神经功能康复,临床效果显著,临床应用价值较高。

Objective To explore the effect of stereotactic hematoma aspiration combined with urokinase in the treatment of hypertensive cerebral hemorrhage patients.Methods From January 2017 to October 2019,40 patients with hypertensive intracerebral hemorrhage admitted to the hospital were selected as the research object,and 20 cases were divided into the control group(craniotomy),and 20 cases in the observation group(using Stereotactic hematoma aspiration combined with urokinase treatment).The clinical effects,neurological deficits,complications,and clinical indicators of the two groups of patients were compared.Results The score of neurological deficits in the observation group after treatment was(11.3±1.3)points lower than that of the control group(24.6±2.4)points,with statistically significant difference(t=26.689,P<0.05).The effective rate of treatment in the observation group was 95.00%higher than 60.00%in the control group(χ^2=7.025,P=0.008).After treatment,the area of edema beside hematoma in the observation group was(7.16±1.32)cm2 smaller than(13.72±1.45)cm2 in the control group,and the hematoma disappeared time was(3.28±1.12)d shorter than that in the control group(9.39±3.46)d,and the difference was statistically significant(t=18.324,21.251,P<0.05).The incidence of complications in the observation group was 5.00%lower than the 35.00%in the control group(χ2=3.906,P<0.05).Conclusion The treatment of hypertensive cerebral hemorrhage patients with stereotactic hematoma aspiration combined with urokinase can effectively reduce their neurological damage,reduce the area of edema adjacent to the hematoma,and reduce postoperative complications,which can better promote the rehabilitation of neurological function.The effect is remarkable and the clinical application value is high.
