摘要
目的探究葡萄糖-6-磷酸脱氢酶(G6PD)、血管内皮生长因子(VEGF)基因在2型糖尿病(T2DM)患者中的表达及临床价值。方法选取陆军军医大学第一附属医院2016年8月-2019年9月收治的T2DM患者124例作为观察组,包括糖尿病肾病(DN)患者48例、非DN患者76例,另选取同期健康体检者120人作为对照组。检测外周血单个核细胞中G6PD mRNA、VEGF mRNA含量。比较两组G6PD mRNA、VEGF mRNA含量,分析两者之间关系,并对比不同DN分期患者G6PD mRNA、VEGF mRNA、肾功能指标[估算肾小球滤过率(eGFR)、尿白蛋白排泄率(UAER)],评价G6PD mRNA、VEGF mRNA与肾功能的相关性。结果 DN患者G6PD mRNA低于非DN患者和对照组,VEGF mRNA高于非DN患者和对照组,差异均有统计学意义(P<0.05)。Pearson相关性分析,G6PD mRNA与VEGF mRNA呈负相关(r=-0.528,P<0.05)。G6PD mRNA随DN分期增加呈降低趋势,VEGF mRNA呈升高趋势(P<0.05)。eGFR随DN分期增加呈降低趋势,UAER呈升高趋势(P<0.05)。Pearson相关性分析,G6PD mRNA与eGFR呈正相关(r=0.599,P<0.05),与UAER呈负相关(r=-0.384,P<0.05);VEGF mRNA与eGFR呈负相关(r=-0.472,P<0.05),与UAER呈正相关(r=0.370,P<0.05)。结论 G6PD、VEGF基因在T2DM中呈异常表达,两者互为关联,且与DN病情、肾功能密切相关,可为临床诊治提供更多依据。
Objective To investigate the expression and clinical value of glucose-6-phosphate dehydrogenase(G6 PD) and vascular endothelial growth factor(VEGF) genes in patients with type 2 diabetes mellitus(T2 DM).Methods 124 patients with T2 DM treated in our hospital from August 2016 to September 2019 were selected as the observation group,including76 non-diabetic nephropathy(DN) patients and 48 DN patients,another 120 healthy people in the same period were selected as the control group.The contents of G6 PD mRNA and VEGF mRNA in peripheral blood mononuclear cells were measured.The contents of G6 PD mRNA and VEGF mRNA in the two groups were compared,and the relationship between the two were analyzed,the DN G6 PD mRNA,VEGF mRNA,renal function indicators [estimated glomerular filtration rate(eGFR),urinary albumin excretion rate(UAER)] in patients with different DN stages were compared,and the correlation between G6 PD mRNA,VEGF mRNA and renal function were evaluated.Results The G6 PD mRNA of DN patients was lower than that of non-DN patients and control group,and the VEGF mRNA was higher than that of non-DN patients and control group(P<0.05).Pearson correlation analysis showed that G6 PD mRNA was negatively correlated with VEGF mRNA(r=-0.528,P<0.05);G6 PD mRNA showed a decreasing trend with increasing DN stage,and VEGF mRNA showed an increasing trend(P<0.05);the eGFR decreased with the increase of DN stage,and UAER increased(P<0.05).Pearson correlation analysis,G6 PD mRNA was positively correlated with eGFR(r=0.599,P<0.05),and negatively correlated with UAER(r=-0.384,P<0.05),VEGF mRNA was negatively correlated with eGFR(r=-0.472,P<0.05),and positively correlated with UAER(r=0.370,P<0.05).Conclusion G6 PD and VEGF genes were abnormally expressed in T2 DM,and they were related to each other and closely related to DN disease and renal function,which could provide more ways for clinical diagnosis and treatment.
