多学科团队合作下老年共病患者的肠内营养管理

Multidisciplinary team based-Enteral Nutrition management in elderly patients with multimorbidities

目的:研究多学科团队合作的肠内营养管理模式在老年共病患者中的应用效果。方法:将神经内科ICU老年共病患者分为对照组和观察组,对照组接受常规营养管理,观察组在对照组的基础上实施多学科合作的肠内营养管理。结果:观察组的肠内营养启动时长较对照组缩短(P<0.001),喂养中断率低于对照组,且喂养7 d达标率高于对照组(P<0.05),观察组14d后的Hb、Alb、CRP均优于对照组(P<0.001);观察组的腹泻、呕吐、误吸发生率均低于对照组(P<0.05)。结论:利用多学科合作、综合干预措施对老年共病患者进行肠内营养管理,有助于肠内营养的早期启动,改善营养相关指标;充分发挥团队中护士的作用,有助于提高肠内营养耐受性,减少并发症的发生。

Objective:To explore the effect of multidisciplinary teamwork-based Enteral Nutrition(EN)management model in elderly patients with multimorbidities.Methods:The elderly patients with multimorbidities in neurolgy department were included and divided into a control group and an observation group,and the multidisciplinary teamwork-based EN management model was implemented in the observation group.Results:The start-up time of EN in the observation group was shorter than that in the control group(P<0.001),the incidence of feeding interruption was lower than that of the control group,and the control rate of 7-day feeding was higher than that of the control group(P<0.05);the level of Hb,Alb and CRP after 14 days was better than those in the control group(P<0.001);the incidence of diarrhea,vomiting,and aspiration in the observation group were lower than those in the control group(P<0.05).Conclusion:Multidisciplinary teamwork-based EN management model in elderly patients with multimorbidities was beneficial to improve the early initiation of EN and control the nutrition-related indicators.Nurses working in their full role in the team could beneficial to improve EN tolerance and reduce the complications.
