羟苯磺酸钙对早期糖尿病肾病肾损伤和T细胞免疫功能的影响 被引量:3

摘要 目的探讨羟苯磺酸钙对早期糖尿病肾病肾损伤和T细胞免疫功能的影响。方法选择2016年8月~2018年8月我院肾病科门诊及住院收治的80例早期糖尿病肾病患者作为研究对象,随机分为观察组和对照组,各40例,均给予基础治疗,对照组采用ACEI(ARB)治疗,观察组此基础上给予羟苯磺酸钙治疗;比较两组24 h尿白蛋白排泄率(24hUAER)、校正后肾小球滤过率(eGFR)及T细胞免疫功能(CD4^+、CD8^+、CD4^+/CD8^+)。结果治疗后两组24hUAER、eGFR均降低,且观察组低于对照组,差异有统计学意义(P<0.05);观察组CD4^+、CD4^+/CD8^+水平分别为(54.15±5.26)%、(2.21±0.97),高于对照组的(46.19±3.91)%、(1.47±0.73),差异有统计学意义(P<0.05);观察组CD8+水平(27.57±2.43)%,低于对照组的(34.55±3.16)%,差异有统计学意义(P<0.05)。结论羟苯磺酸钙治疗早期糖尿病肾病可以独立于降血压、降血糖、降血脂,有效改善患者尿蛋白及肾小球滤过率,调节患者免疫功能。 Objective To investigate the effect of calcium dobesilate on renal injury and T cell immune function in early diabetic nephropathy.Methods We selected 80 patients with early diabetic nephropathy admitted in the outpatient and inpatient nephrology department of our hospital from August 2016 to August 2018 as the research objects.They were randomly divided into observation group and control group,40 cases in each group,all received basic treatment,and the control group ACEI(ARB)treatment,the observation group was given calcium dobesilate on this basis;the 24 h urinary albumin excretion rate(24hUAER),corrected glomerular filtration rate(eGFR)and T cell immune function(CD4^+,CD8^+,CD4^+/CD8^+).Results After treatment,the 24hUAER and eGFR of the two groups were reduced,and the observation group was lower than the control group,the difference was statistically significant(P<0.05);the CD4^+,CD4^+/CD8^+levels of the observation group were(54.15±5.26)%and(2.21±0.97),higher than the control group(46.19±3.91)%,(1.47±0.73),the difference was statistically significant(P<0.05);the observation group CD8+level(27.57±2.43)%,lower than the control group(34.55±3.16)%,the difference was statistically significant(P<0.05).Conclusion Calcium dobesilate in the treatment of early diabetic nephropathy can be independent of lowering blood pressure,lowering blood sugar,and lowering blood lipids,effectively improving the patient's urine protein and glomerular filtration rate,and regulating the patient's immune function.
