摘要
目的探讨血清神经元特异性烯醇化酶(NSE)水平与脑电图对脓毒症脑病(SE)患者预后的评估价值。方法选取2016年6月至2018年10月该院ICU收治的SE患者128例作为SE组,同时选取普通脓毒症患者56例作为对照组。分析2组血清NSE水平变化及脑电图分级情况。结果SE组住院期间死亡29例(22.7%),存活99例(77.3%)。对照组住院期间死亡9例(16.1%),存活47例(83.9%)。2组临床预后比较,差异无统计学意义(P=0.310)。SE组血清NSE水平为(13.86±8.47)U/mL,高于对照组的(5.63±2.91)U/mL,差异有统计学意义(P<0.05)。不同脑电图级别SE患者血清NSE水平比较,差异有统计学意义(P<0.05)。2组死亡患者血清NSE水平明显高于存活患者,差异有统计学意义(P<0.05)。SE组中死亡及存活患者血清NSE水平分别高于对照组中死亡及存活患者,差异有统计学意义(P<0.05)。结论血清NSE水平和脑电图分级与脓毒症患者预后紧密相关,可作为SE严重程度的评估指标。
Objective To investigate the prognostic value of serum neuron specific enolase(NSE)and electroencephalogram(EEG)in patients with sepsis encephalopathy(SE).Methods From June 2016 to October 2018,128 SE patients in ICU of the hospital were selected as SE group,and 56 patients with general sepsis were selected as control group.The changes of serum NSE level and EEG grading of the two groups were analyzed.Results In the SE group,29(22.7%)patients died and 99(77.3%)patients survived.In the control group,9(16.1%)patients died and 47(83.9%)patients survived.There was no significant difference in clinical prognosis between the two groups(P=0.310).The serum NSE level of the SE group was(13.86±8.47)U/mL,which was higher than that of the control group(5.63±2.91)U/mL(P<0.05).There were significant differences in serum NSE levels among SE patients with different EEG levels,and the difference was statistically significant(P<0.05).The serum NSE level of the dead patients was significantly higher than that of the survival patients in the two groups,and the differences were statistically significant(P<0.05).The serum NSE levels of the death and survival patients in the SE group were higher than those of the death and survival patients in the control group respectively,and the differences were statistically significant(P<0.05).Conclusion The serum NSE level and EEG classification are closely related to the prognosis of sepsis patients,which can be used as the evaluation index of SE severity.
