摘要
目的探讨静息心电图对冠心病心肌缺血的诊断价值。方法选取2018年1月~2020年3月于上饶市立医院就诊的80例疑似冠心病心肌缺血患者作为研究对象,所有患者均行静息心电图检查,以冠状动脉造影术为“金标准”,分析静息心电图对冠心病心肌缺血的诊断价值。结果静息心电图诊断冠心病心肌缺血的准确性为91.25%(73/80),灵敏度为94.64%(53/56),特异度为83.33%(20/24),阳性预测值为92.98%(53/57),阴性预测值为86.96%(20/23);经一致性Kappa检测,静息心电图检查冠心病心肌缺血的结果与冠状动脉造影术结果一致性较高(Kappa=0.789,P=0.000);绘制ROC曲线,结果显示静息心电图检查用于冠心病心肌缺血诊断中的AUC为0.835,诊断价值较好。结论静息心电图用于冠心病心肌缺血的诊断中,与冠状动脉造影术结果一致性较高,诊断价值较好。
Objective To investigate the diagnostic value of resting electrocardiogram in myocardial ischemia of coronary heart disease(CHD).Methods A total of 80 patients with suspected myocardial ischemia with CHD who were treated in Shangrao Municipal Hospital from January 2018 to March 2020 were selected as the research subjects.All patients underwent resting electrocardiography,and coronary angiography was used as the"gold standard".The diagnostic value of resting electrocardiogram on myocardial ischemia with CHD was analyzed.Results The accuracy of resting electrocardiogram in diagnosis of myocardial ischemia with CHD was 91.25%(73/80),sensitivity was 94.64%(53/56),and specificity was 83.33%(20/24).The positive predictive value was 92.98%(53/57),and the negative predictive value was 86.96%(20/23).By consistent Kappa test,resting electrocardiogram on the examination of myocardial ischemia with CHD was highly consistent with coronary angiography results(Kappa=0.789,P=0.000).By drawing receiver operating characteristic curve,the area under the curve(AUC)value of resting electrocardiogram for the diagnosis of myocardial ischemia with CHD was 0.835,with a good diagnostic value.Conclusion Resting electrocardiogram used to diagnose myocardial ischemia with CHD is highly consistent with the results of coronary angiography and has good diagnostic value.
