药师对延伸处方中老年人降压药物用药安全干预及效果分析

Analysis of the effect of pharmacists’ intervention on the safety of antihypertensive drugs for the elderly in extended prescription

目的探讨药师对延伸处方中老人高血压用药安全干预的效果。方法选择某2家社区卫生服务中心开具延伸高血压处方用药老年患者120例,药师通过药学门诊开展高血压用药信息调查,了解患者高血压用药依从性情况、服药时间及用法用量正确情况以及注意事项、药品不良反应等项目知晓、对药师服务的满意度情况,并有针对性地进行高血压病健康及安全用药宣教。间隔1个月和2个月后分别对患者上述信息进行追踪随访,了解干预效果。结果两次宣教干预后患者用法、用量知晓率100%,用药依从性、服药时间正确率、主要注意事项和药品不良反应知晓、药学服务满意度均取得显著效果(P<0.001)。结论药师对延伸处方中老人高血压用药宣教干预并持续追踪,可以提升患者的用药依从性,促进用药安全性,提升患者对药学服务的满意度。

Objective To explore the effect of pharmacists’ intervention on the safety of antihypertensive drugs for the elderly in extended prescription.Methods A total of 120 elderly patients with extended hypertension prescription issued by two community health service centers were selected.Pharmacists investigate the hypertension medication information of the enrolled patients to understand the compliance of the patients with the use of antihypertensive drugs,whether the medication time and dosage are correct,how much the patients know about the medication precautions,adverse drug reactions and other items,and how satisfied they are with the service of pharmacists.Then,pharmacists carried out health education on hypertension and safety medication for the patients.The patients were followed up one month later and two months later to understand the effect of intervention.Results The awareness rate of usage and dosage was 100% after twice education interventions,and significant effects were achieved in compliance with medication,accuracy rate of medication time,main points for attention,awareness of adverse drug reactions and satisfaction of pharmaceutical care (P<0.001).Conclusion Pharmacists’ intervention and continuous follow-up on medication of hypertension in extended prescription can improve the elderly patients’ drug compliance,drug safety and satisfaction of pharmaceutical care.
