杭州市萧山区959例儿童手外伤分析

Analysis on 959 cases of hand injury in children in Xiaoshan District of Hangzhou

目的探讨杭州市萧山区儿童手外伤的发生原因并分析其特点,为制定有效预防措施及治疗提供依据。方法选取萧山区2014年1月—2019年12月收治的959例手外伤儿童为研究对象,对其临床资料进行回顾性分析,并对性别、年龄、受伤时间、监护人文化程度、受伤部位、受伤原因等进行总结。结果萧山区共有959例手外伤儿童,男童居多,0~4岁发病率最高,0~4岁切割伤的占比明显高于其他各年龄组各受伤原因,差异有统计学意义(χ^2=44.14,P<0.05)。男童切割伤的占比明显高于女童各受伤原因组,差异有统计学意义(χ^2=11.36,P<0.05)。最易受伤季节为夏季,切割伤占比明显高于其他季节各受伤原因组,差异有统计学意义(χ^2=45.39,P<0.05)。时间段集中在7:00-11:00,监护人文化程度高中以下占多数,受伤部位多在掌指关节远端。结论儿童手外伤与家庭安全教育关系密切,受伤比例与监护人文化程度成反比,应加强夏季上午时间段监督管理,尤其是预防切割伤的发生,一旦发生应及早治疗,改善其预后。

Objective To investigate the causes of hand injury in children in Xiaoshan District, Hangzhou, and to analyze the characteristics, so as to provide basis for the formulation of effective preventive measures and treatment. Methods A total of 959 children with hand injury who were treated in Xiaoshan District from January 2014 to December 2019 were selected as the research objects, the clinical data were retrospectively analyzed, and their gender, age, time of injury, guardian’s education level, injury site and injury cause were summarized. Results There were 959 children with hand injury in Xiaoshan District, most of them were boys, the highest incidence rate concentrated in 0-4 years old, the proportion of cutting injury in children of 0-4 years old was significantly higher than the other injury causes in the other age groups, the difference was statistically significant(χ^2=44.14, P<0.05). The proportion of boys with cutting injury was also significantly higher than girls with other injury causes, the difference was statistically significant(χ^2=11.36, P<0.05). The most vulnerable season was summer, and the proportion of cutting injuries was significantly higher than the other injury causes in other seasons, the difference was statistically significant(χ^2=45.39, P<0.05). The time period was from 7:00 to 11:00, most of the guardians had education level below senior high school, the injured part was mostly at the distal metacarpophalangeal joint. Conclusion Children’s hand injury is closely related to family safety education, and the proportion of injuries is inversely proportional to the educational level of guardians, it is necessary to strengthen the supervision and management in summer morning, especially to prevent the occurrence of cutting injury, once it occurs, it should be treated as soon as possible to improve its prognosis.
