养心通络汤联合氯吡格雷对急性冠心病心绞痛患者血流动力学的影响

Effect of Yangxintongluo decoction combined with clopidogrel on hemodynamics in patients with acute coronary heart disease and angina pectoris

目的探讨养心通络汤联合氯吡格雷对急性冠心病心绞痛患者血流动力学的影响。方法选取乌鲁木齐市中医医院心内科2017—2018年收治的急性冠心病心绞痛患者80例,根据随机数表法分为对照组和观察组,各40例。对照组给予氯吡格雷治疗,观察组在对照组基础上给予养心通络汤治疗。比较2组临床疗效,治疗前后平均血流速度(Vm)、脉动指数(PI)、阻力指数(RI)、心绞痛次数、硝酸甘油用量,并比较2组不良反应发生率。结果观察组总有效率高于对照组(P<0.05)。治疗前2组Vm、PI、RI比较,差异无统计学意义(P>0.05);治疗后观察组Vm、PI、RI低于对照组(P<0.05)。治疗前2组心绞痛次数、硝酸甘油用量比较,差异无统计学意义(P>0.05);治疗后观察组心绞痛次数、硝酸甘油用量低于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论养心通络汤联合氯吡格雷治疗急性冠心病心绞痛的临床疗效确切,可改善其血流动力学指标,也可有效减少心绞痛次数及硝酸甘油的用量,且安全性较高。

Objective To investigate the effect of Yangxintongluo decoction combined with clopidogrel on hemodynamics in patients with acute coronary heart disease and angina pectoris.Methods A total of 80 cases of patients with acute coronary heart disease and angina pectoris were selected from 2017 to 2018 in the Department of Cardiology of Urumqi Hospital of Traditional Chinese Medicine,which were divided into control group and observation group according to random number table,40 cases in each group.The control group was given clopidogrel treatment,and the observation group was given Yangxintongluo decoction on the basis of the control group.The clinical efficacy,mean blood flow velocity(VM),pulsatility index(PI),resistance index(RI),angina pectoris times,and the dosage of nitroglycerin before and after treatment were compared between the two groups.The incidence of adverse reactions was compared between the two groups.Results The total effective rate of the observation group was higher than the control group(P<0.05).Before treatment,there was no significant difference in VM,PI and RI between the two groups(P>0.05);after treatment,VM,PI and RI in the observation group were lower than the control group(P<0.05).Before treatment,there was no significant difference in the number of angina pectoris and the dosage of nitroglycerin between the two groups(P>0.05);after treatment,the number of angina pectoris and the dosage of nitroglycerin in the observation group were lower than the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Yangxintongluo decoction combined with clopidogrel in the treatment of acute coronary heart disease angina pectoris clinical curative effect is exact,it can improve its hemodynamic index,can also effectively reduce the number of angina pectoris and the dosage of nitroglycerin,and with high safety.
