超声引导下腹横肌平面阻滞用于剖宫产术后镇痛的有效性分析与研究

Analysis and Research on the Effectiveness of Ultrasound-guided Transversus Abdominis Plane Block for Post-cesarean Analgesia

目的探究超声引导下腹横肌平面阻滞在剖宫产术后镇痛中的应用有效性。方法回顾性分析2016年12月—2019年12月在该院实施剖宫产手术的60例患者的临床资料,用数字随机分方法将所有研究对象分成对照组和观察组,每组患者30例,术前对两组患者施腰硬联合麻醉,术后观察组患者接受超声引导下双侧TAP阻滞,两组患者术后均接受静脉镇痛泵镇痛,对比两组研究对象的镇痛效果。结果术后2、4、12、24 h观察组患者的疼痛评分均低于对照组,差异有统计学意义(P<0.05),观察组患者镇痛过程中对镇痛泵的按压次数为(8.72±1.66)次,舒芬太尼用量为(44.37±7.14)μg,与对照组的(14.37±2.83)次、(63.29±7.73)μg对比差异有统计学意义(P<0.05)。结论超声引导下腹横肌平面阻滞在剖宫产术后镇痛中的应用有效性较高,降低患者的疼痛程度,减少镇痛泵按压次数和舒芬太尼使用量,值得临床上的推广应用。

Objective To explore the effectiveness of ultrasound-guided transversus abdominis plane block in postoperative analgesia.Methods Retrospectively analyze the clinical data of 60 patients who underwent cesarean section in the hospital from December 2016 to December 2019.All the research subjects were divided into a control group and an observation group using a numerical randomization method,with 30 patients in each group.The two groups of patients were treated with combined spinal-epidural anesthesia before the operation.The postoperative observation group received ultrasound-guided bilateral TAP block.The analgesic effect of the group of study subjects.Results The pain scores of patients in the observation group were lower than those in the control group at 2 h,4 h,12 h,and 24 h after surgery,and the difference was statistically significant(P<0.05).The pressure on the analgesic pump in the observation group was The number of times was(8.72±1.66)times,and the dosage of sufentanil was(44.37±7.14)μg,which was statistically significantly different from the control group's(14.37±2.83)times and(63.29±7.73)μg(P<0.05).Conclusion Ultrasound-guided transversus abdominis plane block is more effective in the application of postoperative analgesia,reducing the degree of pain in patients,reducing the number of analgesic pump compressions and the use of sufentanil,which is worth clinical promotion and application.
