会阴侧切术和适度会阴保护技术对初产妇盆底功能的影响比较 被引量:3

Comparison of the Effects of Lateral Perineal Resection and Moderate Perineal Protection Technique on Pelvic Floor Function of Primipara

摘要 目的比较会阴侧切术和适度会阴保护技术对初产妇盆底功能的影响。方法选取2018年5月—2019年5月该院经阴道分娩初产产妇1600名,随机分对照组和观察组各800名,对照组采用会阴侧切分娩,观察组适度会阴保护技术分娩,比较分娩效果。结果观察组性交痛8.13%、POP下降9.25%、尿失禁1.75%发生率均低于对照组(χ2=6.637、27.978、9.992,P<0.05);产后42 d、产后3个月、产后6个月观察组的阴道动态压力值、POP-Q评分及VAS评分分别为(79.48±2.51)cmH2O、(85.48±2.54)cmH2O、(96.48±1.60)cmH2O、(4.15±1.64)分、(3.61±1.30)分、(3.15±0.45)分、(2.05±0.33)分、(1.87±0.13)分、(1.35±0.54)分明显优于对照组,差异有统计学意义(t=40.236、82.748、113.917、13.918、17.739、42.933、46.631、76.962、33.505,P<0.05)。结论适度会阴保护技术可降低初产妇产后不良事件发生率,改善盆底、阴道收缩能力,降低疼痛,值得推广。 Objective To compare the effects of lateral perineal resection and moderate perineal protection on pelvic floor function of primiparas.Methods From May 2018 to May 2019,1600 primiparous women who gave birth via vagina in the hospital were selected.The control group and the observation group were randomly divided into 800 cases.The control group was delivered by perineal lateral incision,and the observation group was delivered by moderate perineal protection technology.Compare delivery effect.Results In the observation group,dyspareunia was 8.13%,POP decreased by 9.25%,and the incidence of urinary incontinence was 1.75%lower than that in the control group(χ2=6.637,27.978,9.992,P<0.05);42 days postpartum,3 months postpartum,6 cases postpartum vaginal dynamic pressure value,POP-Q score and VAS score of the monthly observation group were(79.48±2.51)cmH2O,(85.48±2.54)cmH2O,(96.48±1.60)cmH2O,(4.15±1.64)points,(3.61±1.30)points,(3.15±0.45)points,(2.05±0.33)points,(1.87±0.13)points,(1.35±0.54)points were significantly better than the control group,the difference was statistically significant(t=40.236,82.748,113.917,13.918,17.739,42.933,46.631,76.962,33.505,P<0.05).Conclusion Appropriate perineum protection technique can reduce the incidence of adverse events in primiparous women,improve the contractility of pelvic floor and vagina,and reduce pain.It is worthy of promotion.
