益气解毒冲剂对单纯疱疹病毒性角膜炎的临床疗效

Clinical Effect of Yiqi Jiedu Granule on Herpes Simplex Keratitis

目的:探讨益气解毒冲剂联合更昔洛韦治疗单纯疱疹病毒性角膜炎(HSK)的临床疗效及对患者T淋巴细胞亚群的影响。方法:选取2015年3月至2016年1月福建省宁德市医院收治的HSK患者148例作为研究对象,按照随机数字表法分为对照组和观察组,每组74例。对照组接受更昔洛韦治疗,观察组在对照组的基础上接受益气解毒冲剂治疗,2组均连续治疗4周。比较2组治疗效果、治疗前后2组外周血T淋巴细胞亚群比例及炎性因子水平;治疗后定期随访6个月,统计2组复发情况。结果:观察组总有效率为98.65%,高于对照组的87.84%(P<0.05);治疗后观察组CD4+细胞比例、CD4+/CD8+较治疗前升高(P<0.01),CD8+细胞比例较治疗前下降(P<0.01),且2组间差异有统计学意义(P<0.01);治疗后2组细胞白细胞介素-18(IL-18)和γ-干扰素(IFN-γ)水平均升高(P<0.05或P<0.01),且观察组高于对照组(P<0.01);观察组复发率为14.29%,低于对照组的45.46%(P<0.05)。结论:在更昔洛韦基础上联合益气解毒冲剂治疗HSK的临床效果更明显,可降低体内炎性因子水平,提高患者细胞免疫功能,且复发率低,疗效显著。

Objective:To investigate the clinical effect of Yiqi Jiedu Granule combined with ganciclovir on herpes simplex keratitis(HSK)and its influence on T lymphocyte subsets.Methods:A total of 148 patients with HSK admitted to Ningde Hospital were selected as the research objects and were randomly divided into a control group and an experimental group according to random number table,with 74 cases in each group.The control group received ganciclovir,while the observation group received Yiqi Jiedu Granule on the basis of the control group.Both groups were treated continuously for 4 weeks.The treatment effects of the 2 groups,the ratio of T lymphocyte subsets in the peripheral blood and the levels of inflammatory factors in the 2 groups before and after treatment were compared;the regular follow-up for 6 months after the treatment was performed,and the recurrence of the 2 groups was counted.Results:The total effective rate of experimental group was 98.65%,which was higher than that of ganciclovir group(87.84%,P<0.05).After treatment,the ratio of CD4+cells and CD4+/CD8+cells in the observation group increased(P<0.01),while the ratio of CD8+cells decreased than that before treatment(P<0.01),with significant difference between the 2 groups(P<0.01).After treatment,the levels of interleukin-18(IL-18)and interferon-γ(IFN-γ)increased in both groups(P<0.05 or P<0.01).And the observation group was higher than the control group(P<0.01).The recurrence rate of experimental group was 14.29%,which was lower than that of the ganciclovir group(45.46%,P<0.05).Conclusion:The clinical effect of ganciclovir combined with Yiqi Jiedu Granule in the treatment of HSK is more obvious,which can reduce the level of inflammatory factors in vivo and improve the cellular immune function of patients with low recurrence rate and remarkable curative effect.
