白癜风患者可溶性白细胞介素-2受体、免疫功能及甲状腺功能的紊乱情况研究 被引量:2

摘要 目的探讨白癜风患者可溶性白细胞介素-2受体(sIL-2R)、免疫功能及甲状腺功能水平。方法选取2016年1月至2019年6月在连云港市第二人民医院治疗的白癜风患者177例(观察组),同时选取健康志愿者100例作为对照组,检测各组sIL-2R、CD3^+、CD4^+、CD8^+、游离三碘甲腺原氨酸(FT3)、游离甲状腺素(FT4)和促甲状腺素(TSH)水平。结果观察组CD4^+T淋巴细胞水平为(36.05±9.92)%,明显低于对照组(P<0.05);而sIL-2R、CD8^+T淋巴细胞和TSH水平分别为(530.02±100.11)/ml,(27.10±7.72)%和(2.80±0.66)mIU/L,均明显高于对照组(P<0.05)。观察组进展期患者CD4^+T淋巴细胞水平为(34.49±8.83)%,明显低于稳定期患者(P<0.05);sIL-2R为(566.11±92.22)/ml,明显高于稳定期(P<0.05);观察组进展期和稳定期患者CD3^+、CD8^+、FT3、FT4和TSH水平比较差异无统计学意义(P>0.05)。观察组寻常型患者sIL-2R和TSH水平分别为(543.08±101.11)/ml和(2.91±0.32)mIU/L,明显高于节段型患者(P<0.05);观察组寻常型和节段型患者CD3^+、CD4^+、CD8^+、FT3和FT4水平比较差异无统计学意义(P>0.05)。结论白癜风患者sIL-2R上调,免疫功能及甲状腺功能明显紊乱,且与疾病分型、分期有一定关系。 Objective To investigate the immune function and thyroid function in the patients with vitiligo.Methods One hundred and seventy-seven patients with vitiligo treated in the hospital from January 2016 to June 2019 were designated as the observation group.At the same time,100 healthy volunteers were enrolled and designated as the control group.The levels of soluble interleukin-2 receptor(sIL-2 R),CD3^+,CD4^+,CD8^+,free triiodothyronine(FT3),free thyroxine(FT4)and thyrotropin(TSH)were detected in the 2 groups.Results The level of CD4^+T lymphocyte in the observation group(36.05±9.92)%was significantly lower than that in the control group(P<0.05),while sIL-2 R,CD8^+T lymphocyte and TSH levels were(530.02±100.11)/ml,(27.10±7.72)%and(2.80±0.66)mIU/L,which were significantly higher than those in the control group(P<0.05).The level of CD4^+T lymphocyte in the advanced-stage patients in the observation group was(34.49±8.83)%,which was obviously lower than that in the stable stage patients(P<0.05),while the sIL-2 R level was(566.11±92.22)/ml,which was significantly higher than that in the stable stage patients(P<0.05).There were no significant differences in CD3^+,CD8^+,FT3,FT4 and TSH,when comparisons were made between the patients in the advanced and stable stages in the observation group(P>0.05).The sIL-2 R and TSH levels in the vulgar patients in the observation group were respectively(543.08±101.11)/ml and(2.91±0.32)mIU/L,which were significantly higher than those in the segmental patients(P<0.05).There were no significant differences in the levels of CD3^+,CD4^+,CD8^+,FT3 and FT4,when comparisons were made between the vulgar and segmental patients in observation group(P>0.05).Conclusion The sIL-2 R level in the patients with vitiligo was up-regulated,resulting in obvious disorder of immune and thyroid function,which might be associated with the type and stage of the disease.
