摘要 目的探讨2019冠状病毒病(COVID-19)患者病程中IgM和IgG抗体水平动态变化趋势.方法采用磁微粒化学发光法新型冠状病毒IgM/IgG检测试剂盒,对来自重庆医科大学附属永川医院感染专科病房2020年1-3月90例新型冠状病毒核酸检测阳性的COVID-19确诊病例的273份血清样本及99份健康医务人员血清标本,进行新型冠状病毒IgM和IgG的检测.结果COVID-19发病后IgM和IgG阳性率随发病时间的推移而逐渐上升,9~15 d达到峰值后回落.99份健康医务人员血清标本检测特异度为100%.Wilcoxon秩和检验分析结果显示,发病当天两组间IgM和IgG具有显著差异(P<0.05).结论新型冠状病毒IgM和IgG抗体检测具有较高的灵敏度和特异度,在发病后9~15 d检测,具有很高的阳性率;新型冠状病毒IgM和IgG抗体可作为对新型冠状病毒核酸检测阴性疑似病例的补充检测指标或协同使用指标. Objective To investigate the dynamic changes of IgM and IgG antibody levels in COVID-19 patients.Methods Magnetic particle chemiluminescence SARS-CoV-2 IgM and IgG test kit was used to assay SARS-CoV-2 IgM and IgG antibodies in 273 serum samples taken from 90 confirmed COVID-19 cases with positive results of SARS-CoV-2 nucleic acid test and 99 serum samples taken from healthy medical personnel from January to March 2020 in Yongchuan Hospital affiliated to Chongqing Medical University.Results The positive rates of IgM and IgG increased gradually over time after COVID-19 onset,and then decreased after peak on days 9-15.The 99 serum samples of healthy medical personnel showed test specificity of 100%.Wilcoxon rank sum test showed that there were significant differences in IgM and IgG levels between COVID-19 cases and healthy control group(P<0.05)on the day of disease onset.Conclusions High sensitivity and specificity are demonstrated in SARS-CoV-2 IgM and IgG assay.Positive rate peaks on days 9-15 after COVID-19 onset.Serum SARS-CoV-2 IgM and IgG assay is helpful for diagnosis of suspected COVID-19 cases.
