摘要
目的通过检测妊娠期糖尿病患者血清及新生儿脐血中胰高血糖素样肽-1(GLP-1)与二肽基肽酶-IV(DPP-IV)的浓度,探究其在妊娠期糖尿病(GDM)中的作用及对孕妇与新生儿的影响。方法记录孕期血糖控制良好及欠佳的GDM孕妇各30例及30例孕期糖耐量正常孕妇的一般资料,并测定空腹葡萄糖、甘油三脂等生化指标及GLP-1与DPP-IV;待脐带夹夹闭脐带后立即抽取脐静脉血,同法处置。追踪、记录母婴妊娠结局。比较孕妇及新生儿DPP-IV,GLP-1浓度,分析其与妊娠期糖尿病的关系以及是否对妊娠结局产生影响。结果孕期血糖控制欠佳的孕妇较孕期血糖控制良好及正常糖耐量孕妇血清中DPP-IV浓度升高,GLP-1浓度下降;脐血中DPP-IV,GLP-1浓度均无差异;3组样本中脐血中DPP-IV浓度均与母血中DPP-IV呈正相关;脐血样本中GLP-1浓度均接近试剂检测值下限。结论 DPP-IV与GLP-1的改变可能与妊娠期糖尿病本身有关,高血糖可显著增加DPP-IV浓度。
Objective To investigate the effect of glucagon like peptide-1(GLP-1) and dipeptidyl peptidase-IV(DPP-IV) in maternal and neonatal cord blood of gestational diabetes mellitus(GDM) and its effect on pregnant women and newborns.Methods The general data of 30 GDM pregnant women with good and poor blood glucose control during pregnancy and 30 pregnant women with normal glucose tolerance during pregnancy were recorded.The biochemical indexes such as fasting glucose,triglyceride,GLP-1 and DPP-IV were measured.After the umbilical cord was clamped,the umbilical vein blood was drawn immediately for the same treatment.Track and record maternal and infant pregnancy outcomes.Comparison of maternal and neonatal DPP-IV,GLP-1 concentration,to analyze its relationship with gestational diabetes and whether it has an impact on pregnancy outcomes.Results The serum DPP-IV concentration in pregnant women with poor blood glucose control increased and the GLP-1 concentration decreased;there was no difference in the DPP-IV,GLP-1 concentration in umbilical cord blood;The DPP-IV concentration in umbilical cord blood in the three groups was positively correlated with the maternal blood DPP-IV;And the concentration in umbilical blood samples was close to the lower limit of reagent detection value.Conclusion The levels of DPP-IV and GLP-1 is probablely relate to gestational diabetes mellitus itself,which high blood glucose can improve the level of DPP-IV.
