Foley尿管球囊定位在腹腔镜剖宫产瘢痕憩室修补中的临床价值

Clinical value of a Foley catheter to locate the diverticulum in laparoscopic repair of uterine cesarean scar defects

目的探讨Foley尿管球囊定位在腹腔镜瘢痕憩室修补术中的临床价值。方法选择2018年1月~2019年6月期间我院诊治的27例有生育要求或者异常子宫出血的剖宫产瘢痕憩室患者,按手术方法随机分为两组:对照组13例行腹腔镜下剖宫产瘢痕憩室修补术;Foley尿管球囊定位组14例,腹腔镜剖宫产瘢痕憩室修补术前在宫腔镜指示下将Foley尿管插入宫颈口约4~5cm,起到指示定位作用。结果两组患者的手术时间差异无显著性(P>0.05),但球囊组患者的术中出血量低于普通组,差异有显著性(P<0.05)。两组术后TRM都有明显增厚,经期显著缩短,但两组比较,差异无显著性(P>0.05)。结论腹腔镜瘢痕憩室修补术中球囊定位可以更准确定位憩室,并且凸起的球囊使憩室与周围组织的关系更加清晰,从而可缩短手术时间,减少并发症。

Objective To assess clinical value of a Foley catheter to locate the diverticulum in laparoscopic repair of uterine cesarean scardefects(CSD).Methods From January 2018 to June 2019,27 patients with cesarean section scar diverticulum diagnosed and treated in our hospital with fertility requirements or abnormal uterine bleeding were selected and randomly divided into two groups according to the surgical method:13 cases in the control group underwent laparoscopy Uterine scar diverticulum repair;Foley urethral balloon positioning group(experimental group) 14 cases,before laparoscopic cesarean section scar diverticulum repair,the Foley urinary catheter was inserted into the cervix about 4 to 5 cm under the instructions of the hysteroscope.To indicate the positioning function.Results There was no significant difference in the operation time between the two groups(P>0.05),but the intraoperative blood loss of the balloon group was lower than that of the ordinary group,and the difference was significant(P<0.05).After the operation,the TRM thickened and the menstrual period was significantly shortened in both groups,but there was no significant difference between the two groups(P>0.05).Conclusion In laparoscopic repair of cicatricial diverticulum,balloon positioning can more accurately locate the diverticulum,and the protruding balloon can make the relationship between diverticulum and surrounding tissues more clear,which can shorten the operation time and reduce the complications.
